- Physical Evaluation Board Liaison Officer Services Combined Synopsis/Solicitation Amendment FA5613-21-R-0006 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Funds are not currently available for this requirement as the requirement will be funded with FY 2022 funds. See FAR 52.232-18, �Availability of Funds.� (iii) The solicitation number is FA5613-21-R-0006 and the solicitation is issued as a request for proposal (RFP). This acquisition is conducted in accordance with FAR Part 12 and 13 procedures. (iv) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-09/10-26-2020, Defense Acquisition Circular effective 01 October 2020, and Air Force Acquisition Circular AFAC 2019-1001 effective 1 October 2019. (v) This is not a set-aside. The North American Industry Classification System (NAICS) code for this solicitation is 621399. (vi) The 700 CONS/PKA Flight has a requirement to contract for Physical Evaluation Board Liaison Officer Services (Non Personal Service) for the 86th Medical Group�s (86 MDG) at Ramstein Air Base, Germany, for the period 1 December 2021 � 31 November 2022 with four 1 year options through 31 November 2026. Offerors shall use Attachment 1, Pricing Schedule, which includes all contract line item numbers, quantities and units of issue, to fill in all unit prices and total amount blocks. (vii) Attachment 2, Performance Work Statement (PWS), contains the description of the requirement for Physical Evaluation Board Liaison Officer Services for the 86th Medical Group�s (86 MDG). Please note that the position has not been previously approved under NATO Status of Forces Agreement (SOFA) Supplementary Agreement Article 73 and will need prior approval if SOFA status is required. (viii) Places of performance and acceptance is at the 86th Medical Group�s (86 MDG) facilities at Ramstein Air Base. Required period of performance: Base: 1 February 2022 � 31 January 2023 Option Year 1: 1 February 2023 � 31 January 2024 Option Year 2: 1 February 2024 � 31 January 2025 Option Year 3: 1 February 2025 � 31 January 2026 Option Year 4: 1 February 2026 � 31 January 2027 (ix) The provision at FAR 52.212-1, Instructions to Offerors -- Commercial Items (Jun 2020), applies to this acquisition.
