This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. The Government reserves the right to make no award from this solicitation. This solicitation is a request for quotations (RFQ). The government anticipates awarding a firm-fixed price purchase order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-03. The associated North American Industrial Classification System (NAICS) code for this procurement is 511210, Software Publishers, with a small business size standard of $41.5 Million. This solicitation is a 100% Full & Open. Service-Disabled Veteran Owned Small Businesses (SDVOSBs) must be verified in the Vendor Information Pages (VIP), https://www.vip.vetbiz.va.gov/, at the time of receipt of quotes and at the time of award. The Department of Veterans Affairs, Network Contracting Office 5, 510 Butler Avenue, Martinsburg, WV 25405 is soliciting quotations from all vendors to provide HCHT Censitrac Instrument Tracking Software at the VISN05 facility listed below. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information along with any other documents necessary to support the requirements below. All interested companies shall provide quotation(s) for the following: (See Attachment 1). STATEMENT OF WORK Veterans Integrated Service Network -05 (VISN05) STATEMENT OF WORK CENSITRAC INSTRUMENT TRACKING SOFTWARE GENERAL: Scope of Services: The Contractor shall provide the equipment, installation, and services to provide full implementation of the CensiTrac Instrument Tracking System (ITS) at the Martinsburg VA Medical Center. Contractor shall supply all parts, materials, tools, and equipment for the completion of the project. Location: Martinsburg VA Medical Center, 510 Butler Avenue, Martinsburg, WV 25405 The Government anticipates awarding a Firm Fixed Price contract. WORK HOURS: Hours of Operation: Normal business hours for implementation work are 8 a.m. to 4:30 p.m. The Contractor shall provide technical support services via phone twenty-four hours a day, seven days a week. The Contractor shall provide on-site support as needed during normal business hours. The Contractor will be authorized to provide afterhours support as needed with communication to the facility. National Holidays: The ten holidays observed by the Federal Government are: New Year s Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. DEFINITIONS SPS: Sterile Processing Service ITS: Instrument Tracking System CensiTrac: Censis Instrument Tracking System this is the software program name and generally used to refer to the entire system. OIT: Office of Information and Technology for the VA VA: Veterans Affairs SPECIFICATIONS: Equipment/Materials: The ITS shall include VHA OIT approved computers/workstations with waterproof keyboard and touchpad mice. Hardware to mount the computers shall be included. The Contractor shall provide scanners and stands, wireless FIPS compliant scanners, label printers with network capabilities and USB cables, laser get printer, and document scanners. The requirement shall include a camera and stand. The labels for the label printers and some ribbons shall be included. All CensiTrac software, licenses, and subscriptions shall be included. Installation Contractor shall unbox all equipment/supplies/etc. Contractor shall put in place all equipment according to the site visit plan Contractor shall install all brackets/mounts for equipment Contractor shall configure all computers/scanners/printers Contractor shall install all software on computers Training Contractor shall train all SPS staff on-site on using the software and the proper workflow Contractor shall train SPS staff for an additional week on-site upon request of Martinsburg VAMC based on need Instrument Marking All supplies such as electrochemical marking filter caps, electrochemical marking electrolytes, and electrochemical marking neutralizers shall be included. The Contractor shall not mark any instruments that SPS tells them should not be marked. Contractor shall provide updates on marked instruments. Contractor shall train the SPS staff on instrument marking. Safety: The Contractor shall follow federal regulations. The Contractor shall adhere to infection control guidelines. The Contractor must comply with all safety practices and CDC regulations. The Contractor is responsible for reporting any hazards they come across to their point of contact and report emergencies to campus Dispatch/Police via extension 4911 as applicable. Personal Protection Equipment (PPE): Contractors shall be responsible for their own PPE when performing work on-site. Masks shall be worn in all healthcare areas. Temperature Screening: All persons entering the facility shall have their temperature taken by the screening contract employee. Verbal Screening: All persons entering the facility shall be verbally screened by contracted screeners based on current CDC (Center for Disease Control) guidelines. Contract personnel are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. GOVERNMENT FURNISHED ITEMS AND SERVICES Instruments Desks/surfaces CONTRACTOR FURNISHED EQUIPMENT/SUPPLIES All included equipment and supplies as per the above Tools to complete installation PPE QUALITY ASSURANCE The Government will test the end-product to ensure completeness and effectiveness. Performance will be reviewed on a periodic basis by the Contracting Officer s Representative on behalf of the Government. Place of Performance: VISN05 Medical Center. Martinsburg VA Medical Center 510 Butler Avenue Martinsburg, WV, 25405 Hours of Operation: The Martinsburg VAMC requires the contractor to deliver within 30 calendar days from the contract award to the VA medical facility warehouse above during normal business hours from 8:00AM to 3:30PM. (EST), Monday through Friday. Government Responsibilities: No Government Furnished Property (GFP) will be provided. The Government will provide access to areas and an escort when required by participating Medical Center policy. The Government will provide applicable VA Polices, Handbooks, Guides, Regulations and Medical Center Memorandums The Government will unilaterally award Task Orders to fund the required services for each facility. Price Schedule: The contractor must provide a price quote for each CLIN delineated in Attachment 1 to be considered for award. Must provide the authorized distributor letter if you are not the manufacturer. See Attachment 1: Price Quote Contract Line Item Number (CLIN) Table F. Evaluation of Quotes: The Government will select the quote that represents the best benefit to the Government at a price that can be determined reasonable. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. FAR 52.212-2 Evaluation Commercial Items . Price Only and Brand Name Only . Specific quote requirements are detailed in attached FAR 52.212-1 Instructions to Offerors Commercial Items . Following receipt of quotes, the Government will perform an evaluation using a comparative evaluation of the services quoted in accordance with FAR 13.106-2(b)(3). The Government will compare quotes to one another to select the quote that best benefits the Government by fulfilling the requirement. Contractors may submit more than one quote; however, contractors are strongly encouraged to submit their best technical solutions and prices in response to this request for quote. Once the Government determines there is/are a contractor(s) that can provide a product that meet(s) the requirements of this request for quote, the Government reserves the right to communicate with only those contractors quoting the best-suited service to address any remaining issues. G. Clauses and Provisions: The clauses and provisions for this solicitation are found in Attachment 4: Solicitation Clauses and Provisions. The Clauses and Provisions provide additional information regarding the requirements for quote submission and evaluation. This is an open-market combined synopsis/solicitation for supplies as defined herein. The government intends to award firm fixed price contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. Orders will then be placed against this contract. All quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
