- This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for Market Research. This is a Request for Information only. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The Department of Veterans Affairs Audie L. Murphy VA Hospital Pharmacy Service in San Antonio, Texas intends to award a brand name or equal contract award for the purchase of FastPak Service for Audie L. Murphy Medical Center and Kerrville VA Medical Center. The Government is conducting a market survey to help determine the availability and technical capability of qualified service-disabled veteran-owned small businesses, veteran-owned small businesses, small businesses, HUBZone small businesses and/or other large businesses capable of serving the needs identified below. This notice of intent is for open market as well as Federal Supply Schedule items. The purpose of this notice is to gain knowledge of potential qualified sources and their size classification/socioeconomic status (service-disabled veteran owned small business, veteran owned small business, women owned small business, HUB Zone, 8(a), small business or large business, relative to NAICS 339112 with a size standard of 1000 employees. Responses to this notice will be used by the Government to make appropriate acquisition decisions. A solicitation is not currently available. If a solicitation is issued, it will be announced on Federal Business opportunities website http://www.fbo.gov or GSA E-Buy at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. AUDIE FAST PAK ATP AND FIRST LPU SERIAL NUMBERS: ATP: 20040138; 20060206 LPU: 20040138N; 20060206N ADDITIONAL LPU: 20070222N; 20070225N INSPECTRX: PH119C07 KERRVILLE FASTPAK ATP AND FIRST LPU SERIAL NUMBERS: ATP: 20060201 LPU: 20060201N ADDITIONAL LPU: 80030081N INSPECTRX: PH119E19 WITH BASE YEAR PRICING IS FOR THE PERIOD OF 11/15/2021- 11/14/2022 WITH THE OPTION TO EXERCISE. STATEMENT OF WORK (SOW) DESCRIPTION OF SERVICES: This Statement of Work (SOW) is to establish Services for Basic Full-Service maintenance for FastPak unit dose packaging system equipment for Kerrville VA Medical Center and Audie Murphy VA Medical Center and to establish services for Bullseye Service Maintenance for San Antonio. The contractor will provide service maintenance services for two (2) FastPak EXP unit dose packaging systems, and two (2) additional Lower Packaging Units (LPUs) at ALMD; one (1) FastPak EXP unit dose packaging system at KD, and one additional Lower Packaging Unit (LPU) at KD for the period of performance. SCOPE OF WORK The contractor will provide Basic Full-Service maintenance as indicated below for FastPak EXP equipment resources at ALMD and KD and Bullseye service maintenance for San Antonio. Telephone Support: During the term of this Agreement, TCGRx will provide Customer access to its Helpline which can be called 24 hours per day, 365 days per year to obtain support. To allow TCGRx to furnish support and collect information regarding the Equipment and Software, Customer shall maintain the data line attached to the Equipment modem in good working order and shall permit TCGRx to transfer electronic data to and from the Equipment at any time for pharmacy dispensing purposes and remote diagnostics in case of performance issues. No patient data will be transferred without consent of Customer. Service: During the term of this Agreement, TCGRx shall perform the service necessary to keep the Equipment and Software in good working order at Customer site subject to the provisions under Misuse of Equipment, below. If the Equipment or Software becomes disabled, Customer will promptly notify TCGRx of the disability and any known cause. Customer shall assist TCGRx in performing any diagnosis and correction of problems via the remote communications interface. In the event the customer is unable to provide remote access and/or assistance from on premise pharmacy personnel, TCGRx, at its discretion, may request compensation to cover the expense of an on-site visit that could have been resolved via remote support with on premise pharmacy personnel assistance. If remote diagnosis indicates, TCGRx will use good faith efforts to respond to on-site service needs within 24 hours. General service repairs occur between the hours of 8Am and 5PM local time; if requested by the customer, there may be an additional charge for work performed during evening, weekend, or holiday hours. Customer shall provide TCGRx access to the Equipment at all reasonable hours to perform necessary service, including preventative maintenance. Service includes adjustments and replacement and repair of parts and/or software as TCGRx deems necessary. Replacement parts may be re-manufactured, equivalent to new in performance, and are furnished on an exchange basis. Parts and on-site labor, including travel, are provided at no additional cost. Subcontracting: TCGRx may subcontract any part or all of its obligations under this Agreement, provided however that any such subcontracting will not relieve TCGRx of its obligations or duties to Customer under this Agreement. 2.5 Use of Information: TCGRx shall be permitted to collect information with regard to Customer's use of the Equipment during the term of this Agreement, including drug utilization and inventory management information, and retain and use such information as its sole and absolute property. TCGRx shall have no right to use any patient or subscriber identifying information. Period and place of performance: 3.1 The place of performance for this requirement will be at the following location: South Texas Veterans Health Care System Audie L. Murphy Medical Center Kerrville VA Medical Center 7400 Merton Minter Boulevard 3600 Memorial Boulevard San Antonio, TX 78229-4404 Kerrville, TX 78028 3.2 Period of Performance will be base year: 11/15/2021 11/14/2022. There will be four (4) option years that could extend the term of the contract; option year one 11/15/2022 11/14/2023; option year two 11/15/2023 11/14/2024; option year three 11/15/2024 11/14/2025; option year four 11/15/2025 11/14/2026. Exercising of Option Years are at the discretion of the Government. Awardee shall have 30 days after award to prepare and begin service. 3.3 Work Hours: Normal business hours are 07:30 a.m. to 16:30 or 15:00-23:00. Monday thru Friday excluding Federal Holidays. Work completed outside this time must be requested through the Contracting Officer s Representative (COR) and approve by the Contracting Officer (CO). Request for after hour work must be submitted in writing to the COR and CO. Coordinator two (2) week prior to work. The VA requires that information submitted must contain: extent of work, worker involved, the affected areas, and the estimated times of operation. 3.4 Employee Badging: All workers are required to obtain a time-limited I.D. badge from the VA Police Service located in the main building of the hospital. This badge must be worn at all times when workers are on site. Background: Use of pharmacy automation systems such as the FastPak Unit-Dose Packaging systems minimize errors and patient wait times and promote patient and pharmacy staff satisfaction. To maintain such systems in fully operational status always in support of patient care activities, it is essential to have a full-service maintenance contract in place. The Full Basic Service maintenance for the FastPak EXP equipment includes on-site visits 8am-5pm Monday through Friday, all parts, 24/7 phone support, and two (2) preventative maintenance visits annually. Performance objectives: The contractor will provide Basic Full-Service maintenance services for FastPak EXP equipment resources as indicated below. 5.1 On site visits 8am-5pm Monday through Friday 5.2 All parts 5.324/7 phone support 5.4 Two (2) preventative maintenance visits annually Operating constraints: There are no operating constraints identified by the South Texas Veterans Health Care Systems. 7.0 List of equipment and quantities that will be cleaned. The certification and accreditation requirements do not apply to this procurement nor is a security accreditation package required. 9.0 Privacy Officer: The contractor will not have access to protected Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage device will remain in VA control and will not be removed from VA custody. All research data available for Contractor analyses is de-identified. 10.0 Records Manager: There will be no federal records created, maintained, used or disposition with this contract; regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.].
