- RFQ # 36C24821Q1428 MEDICAL PHYSICIST INSPECTION SERVICES, Tampa FL. The contractor shall furnish all labor, equipment, transportation, accessories and qualified personnel necessary to perform all mandatory, regulatory, medical physicist inspections/evaluations of radiation emitting devices, Magnetic Resonance Imaging (MRI) scanners and personal protective lead devices at the James A. Haley Veterans Hospital (JAHVA), 13000 Bruce B. Downs Blvd., Tampa, Florida 33612 and its outlying clinics (New Port Richey, South Hillsborough, Lakeland and Primary Care Annex) in accordance with the price/cost schedule and identified on list of devices. On Columbus Day (October 11, 2021) and annually thereafter on Columbus Day, the contractor shall furnish all labor, equipment, transportation, accessories and qualified personnel necessary to perform all mandatory, regulatory, medical physicist annual inspections/evaluations of X-Ray, Fluoroscopic, dental and C-Arm devices (77 pieces of equipment) and up to 750 pieces of personal lead protective equipment at the James A. Haley Veterans Hospital, 13000 Bruce B. Downs Blvd., Tampa, Florida 33612 and its outlying clinics (South Hillsborough, New Port Richey, Lakeland and Primary Care Annex). The contractor must be a subject matter expert of, and adhere to, the Joint Commission (TJC) requirements and standards for medical imaging, conduct performance evaluations in accordance with applicable American College of Radiology (ACR) guidelines and follow relevant VHA Directives, including: 1. VHA Directive 1105.04, Fluoroscopy Safety Dated June 21, 2018 1105_04_D_2018-06-21.pdf 2. VHA Directive 1129, Radiation Protection for Machine Sources of Ionizing Radiation 1129_D_2015_02_10.pdf The Contractor shall comply with radiation protection standards in 29 CFR 1910.1096 and immediately report any unsafe conditions with the potential to adversely impact the facility radiation workers or patients to the Radiation Safety Officer (RSO). The Government may exercise at their discretion, the option to extend the term of the contract for four (4) additional option year periods subject to the required need and the availability of funds. ELIGIBILITY: SDVOSB SET ASIDE This solicitation is set-aside and limited to responsible service-disabled veteran owned small businesses (SDVOSB) with the ability to provide medical physicists radiological inspection services, as described in the Performance Work Statement (PWS). Quotes will be considered only from offerors who are regularly established in the business called for. To be eligible for award, Contractors must be a verified service-disabled veteran owned small business and possess a valid an active System for Award Management (SAM) registration.
