Description
This Socilitation is for TWO (2) Nonpersonal Service Contract for Registered Nurse (RN) Services on ""As Needed Basis"" to provide uninterrupted patient care services to be performed in the Ambulatory Care Clinic (ACC) at Four Corners Regional Health Center (FCRHC) located at HCR 6100, US Hwy 160 & Route 35 Red Mesa, Box 30, Teec Nos Pos, AZ 86514. Registered Nursing Service shall be performed in accordance with the attached Performance Work Statement (PWS) and Position Description. Period of Performance: Date of Award through 12 months. The assignment is through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from contract assignment. Shift: days 0800-1630 or 0900-1730, no weekends and off holidays The Government does not guarantee a specific shift to any individual(s), shift hours shall greatly depend on patient case load.The Government does not guarantee the contractor to work the full number of hours. The Health Center serves approximately 7,000 Native Americans, of which the majority are members of the Navajo Nation. Strong Navajo cultural traditions exist within the community, offering an opportunity to learn the Navajo language, or to learn about traditional Navajo medicine. Recreation opportunities exist such as camping, fishing, hiking, biking, backpacking, snowshoeing, snowboarding, cross country skiing, and alpine skiing. Telluride, Durango, Moab, Canyonlands, Grand Gulch, the Colorado River, the San Juan River, several large lakes, and Colorado Wilderness areas are just short drives away. CONDITION OF EMPLOYMENT: Type of Citizenship: U.S. Citizenship is required. As a Registered Nurse (RN) contractor shall provide prescribed medical treatment and personal care services to American Indian and Alaska Native beneficiaries. Duties include documenting observations, assessments, and changes in patient's condition; collaborating with health team members to facilitate positive patient care outcomes; providing a full range of nursing care to patients with a variety of physical and/or behavior problems; and accountable for meeting and maintaining the qualification standards and functional statements of competency standards. Our facility is seeking individuals who possess good organizational skills which enable completion of work, a demonstrated ability to assume leadership in a patient centered program, and the ability to establish effective communication with patients, families, visitors and members of the interdisciplinary team. Background Investigation: All qualified referred candidates will undergo Fingerprinting process. Fingerprint Card: FBI FD-258 (provided by the Federal Government): Fingerprinting MUST be cleared and approved prior to contact assignment. Contract assignment(s) are conditional on a cleared background check. OPM requires Electronic Questionnaire for Investigation Processing (e-QIP) for registered nursing services exceeding One Hundred Twenty (120) days. Electronic Questionnaire for Investigation Processing (e-QIP): Executive Orders 10450, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM) system. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant subfactors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. Factor 1: Agency's Past Performance Subfactor: Number of similar contracts Subfactor: Timeliness of providing candidate(s) Factor 2: Registered Nurse (RN) Qualifications Requirements Subfactor: Certification (BLS, PALS, ACLS) Subfactor: License/Registration Subfactor: Professional Development Subfactor: General Experience / Specific Experience Subfactor: Equipment (Refer to PWS 6.3) Subfactor: Health Requirements (Medical Evaluation / Immunization) Medical Evaluation: The contractor shall possess a health status allowing physical and mental ability to carry out required functions. This is to be verified on an annual basis by the employing agency or a copy of a physical examination provided by the contractor/practitioner. Immunization: Contractor shall provide evidence of current immunization documentation for MMR (measles, Mumps, and Rubella) Diphtheria and Tetanus and DT Booster, OOD status, or as required by the Navajo Area IHS immunization policy. A copy of the document shall be submitted to the Employee Nurse upon arrival at the hospital before providing service. Factor 3: Registered Nurse (RN) Availability Subfactor: Upon fingerprinting clearance, RN shall be available as soon as possible to start. Subfactor: RN shall be flexible to allow for short term (13-weeks), long term (1-year) and, where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. Factor 4: Professional Nursing Skills: Subfactor: MUST have at a minimum 4 years of recent outpatient, hands on, Ambulatory Care Clinic (ACC) experience �Subfactor: MUST be able to work independently and proficiently after no more than 2 days of orientation Subfactor: MUST have recent hands on Ambulatory Care Clinic experience and be proficient with pediatrics patients Subfactor: MUST have recent Ambulatory Care experience and be proficient with newborns Subfactor: MUST have recent Ambulatory Care experience and be proficient with pediatric immunizations Subfactor: MUST have recent Ambulatory Care experience and be proficient with patient screening and patient room placement. Subfactor: Ability & knowledge to develop patient care plans Subfactor: Ability & knowledge to learn the different types of equipment needed for patient care. Subfactor: Ability & knowledge of medications and adverse reactions. Subfactor: Ability & knowledge to start an IV Subfactor: Ability & knowledge to triage using 5 level triage algorithm Subfactor: Ability & knowledge to give children immunization Subfactor: Ability & knowledge to work with Electronic Health Record Subfactor: Ability & knowledge to float to other departments upon completion of a float competency skills checklist. Housing: Government housing maybe available. Travel to Northern Navajo Medical Center at Highway 491 North, Shiprock, NM 87420 maybe required for indoctrination, training and other work related activities from time-to-time. PROPOSAL SUBMISSION INSTRUCTIONS: 1. Quotation in accordance with the line items above. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The Attach Wage Determination Service Contract Act applies. 2. Past Performance: Provide agency experience of comparable scope and complexity in providing services within the past three (3) years of operation. 3. Qualifications: Provide complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information, Professional Nursing Skills and all relative documents for proposed candidate. Refer to PWS Section 5.0 & 6.0). 4. Availability of candidate(s). NOTE: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. In addition, any profiles received before or after the RFQ dates WILL NOT be accepted.
