Page 3 of 3 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number 36C24821Q1699 is issued as a Request for Quotation (RFQ) in conjunction with FAR -- Part 13 Simplified Acquisition Procedures and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. The provisions and clauses in this document are in effect through the Federal Acquisition Circular 2021-04, January 19, 2021. This solicitation intends to award a contract to a SDVOSB/VOSB business associated with NAICS Code 334510 with a size standard of 1250 Employees. The Miami Veterans Healthcare System has a requirement for audiology equipment/hardware essential to daily operations. This solicitation is for Brand Name or Equal to the salient characteristics and functionality of the following 1) Interacoustics Eclipse EP25 with ABR-OAE and 2) Interacoustics Eclipse EP25 with OAE-VEMP. 1) Interacoustics Eclipse ABR-OAE: This is a diagnostic tool that measures Otoacoustic emissions, the brain s response to sound and vestibular function. The ABR is recommended to determine the possibility of a disorder along the hearing nerve or the brain. The VEMP protocol will be used to identify vestibular lesions. 8510949 Interacoustics Eclipse EP25 1 EA 8506058 Interacoustics Eclipse Laptop 1 EA 8515366 Interacoustic Eclipse DPOAE Module License Code 1 EA 8070 Installation and Training 1 EA 2) Interacoustics Eclipse (OAE-VEMP): This is a diagnostic tool that measures Otoacoustic emissions, the brain s response to sound and vestibular function. Comparative evaluations will be performed after all quotes have been received. SALIENT CHARACTERISTICS: Equipment must have the physical characteristics to enable the government to perform the capabilities: Eclipse Hardware Specifications Eclipse hardware platform, Advanced EP auditory software package, ECochG, ABR (15 & 30 ms), MLR, LLR, P300, MMN, EPA4V Preamp with adjustable neck strap, EPA4 and EPA3 Patient electrode cables, OtoAccess database software , ABR Insert earphones, B71 Bone conductor complete, LBK-15 Loop Back test box (for testing system integrity), Electrode cables, disposable pads, Ear tips, tiptrodes, skin prep, Operation manual & power supply. The system must include the CE-Chirp LS stimuli and the Narrow Band CE-Chirp LS stimuli for short duration ABR testing. All protocols on the system must be fully customizable by the user. The software will include objective measurements Fmp, Residual Noise, Waveform Reproducibility and Wave V signal to Noise Ratio. The Narrow Band CE-Chirp LS will be the stimuli used in the ASSR program.  The ASSR program will allow you to select and record 8 different intensities while testing both ears simultaneously. As each ASSR response is obtained, the intensity can be changed while the remaining frequencies are being tested. The system must be on a windows platform. The system must use OtoAccess database. The system must be expandable to add software applications in the field. The system shall have the ability to export Data files in .xls or MAT format The system shall have the ability to import custom stimuli. The Vemp software will have the ability to test the CW-Chirp stimuli. The Vemp software will include the ability to test Bone Conduction. The Vemp software will provide EMG rectification. The ECoG software will calculate both SP/AP amplitude ratio and Area ratio as well as the latency difference between the AP rarefaction and AP condensation. Bayesian weighted averaging will be available in both Otoacoustic Emission testing. Offeror quotations must demonstrate meeting the salient physical, functional, or performance characteristic specified in this solicitation, otherwise it will be considered non-responsive. The information identified provided is intended to be descriptive, not restrictive and to indicate the quality that will be satisfactory. The interested parties bear full responsibility to ensure their submission demonstrates to the government that they can provide the supplies being requested. Only new equipment is acceptable; no remanufactured, used/refurbished or ""gray market"" items. No product in development shall be considered. All items must be covered by the manufacturer's warranty. At a minimum the quote shall contain the following: The name, address, and telephone number of the vendor. Tax Identification Number (TIN) DUNS Number / Cage Code Point of Contact for submission of orders if awarded a contract Authorized Distributor Letter certified by OEM. Quoted price shall include delivery.
