- STATEMENT OF WORK SCOPE OF WORK 1. General Information: The contractor shall provide Qualification of Elevator Inspectors (QEI)-certified mechanics to serve as third party witnesses to annual and semi-annual safety inspections and testing for eleven (11) passenger elevators located at the Canandaigua VA Medical Center (VAMC), 400 Fort Hill Ave, Canandaigua, NY 14424-1159. The period of performance is from November 1, 2021 October 31, 2022 with four option years. All inspections and testing shall be done according to the most current edition of ASME A17.1, Safety Code for Elevators and Escalators. The inspection and tests shall be executed in a manner that is in accordance with ASME Guide for Inspection of Elevators, Escalators and Moving Walks (ANSI/ASME 17.2), hereafter referred to as the A17.2 Code. All Elevator Inspections and Testing shall be done by QEI-Certified Mechanics. The contractor shall provide a QEI-Certified Mechanic to perform elevator inspections and to be a third-party witness to semi-annual elevator tests. A separate contract shall provide the personnel, labor, supplies, equipment including weights for all tests covered under this procurement. Services are expected to be performed during the months of April and October. However, services shall be coordinated with the Contracting Officer Representative (COR), who will coordinate with the other contractor. 2. Hours of Work: Work required in the performance of this contract shall normally be performed during the Medical Center s regular administrative working hours, 8:00am to 4:30pm, Monday through Friday, exclusive of Federal Holidays. Contractor shall include provision to provide up to 4 hours of testing time to be performed during off-hours (6 pm to 10 pm) to complete load testing in year four and five. This will allow testing in clinical areas with the least amount of disruption to clinical services. Upon arriving at the installation to perform services, the contractor shall report to the Contracting Officer s Technical Representative, before and after performance of service, providing evidence of service performed within 5 business days. Reports REPORT OF TEST OF ELEVATOR GOVERNOR AND SAFETY DEVICES (VAFORM 10-6166, attached) form will be used to document test results. Semi-annual maintenance inspections shall be performed by the contractor on each listed piece of equipment. The Elevator Inspection Report shall be used to document the results of each inspection. In addition to using the standards in the A17.1 and A17.2 Codes, the inspection contractor shall assure that the maintenance contractor s preventative maintenance check charts are up to date and correctly annotated, and shall inspect the Elevator Equipment for evidence of proper maintenance, adjustment and repair. Deficiencies shall be recorded in the remarks area of the inspection form. 6. Elevator testing to be performed: The contractor shall witness and document the following series of tests: a. Hydraulic Elevators (Annual Requirements) aka Pressure Tests: (Total Elevators 2) 1005.2a RELIEF VALVE - Check pressure release to be sure valve opening or pump cut-off pressure is no more than 125 percent of working pressure. 1005.2b CYLINDER LEAKAGE - Locate car at a convenient observation level, and open disconnect switch. After fifteen (15) minutes, minimum time, note position of car as compared to initial level. A drop in elevation beyond that accountable to visible oil leakage or oil temperature change (shrink) will indicate cylinder or buried oil line leakage. 1005.2c ADDITIONAL INSPECTIONS AND TESTS - Following indicated devices shall be examined and tested as required: Normal and Final Terminal Stopping Devices (1002.2e) Simulated Elevator Operation Under Fire or Other Emergency Condition (1002.2f) Simulated Standby (Emergency) Power Operation (1002.2g) Emergency Terminal Speed Limit Device (305.2) b. Traction Elevators (Annual Requirements) aka No-Load Tests: (Total Elevators 9) CAR SAFETIES 1002.3a - Inspect guide clearance, determine all working parts of the car safeties are in satisfactory operating condition, hand test to see safeties operate simultaneously with equal travel. With no load in car, run car down at slowest speed, hand trip governor and bring car to rest on safeties. The safety shall bring the car to rest promptly. GOVERNOR 1002.3b - Inspect and operate by hand to determine that all governor parts operate freely. Inspect bearings, rope grip jaws, and all rubbing surfaces for excessive wear and to be sure they are free of any foreign matter, (paint). OIL BUFFERS 1002.3c - Buffers shall be checked for oil level to be within allowable limits. Plunger shall be fully compressed and shall return to fully extended position within 90 seconds after release. c. Semi Annual Elevator Inspections: (Total Elevators 11) REPORT OF INSPECTION OF ELEVATORS (VAFORM 10-6166) form will be used to document inspection results. d. Traction Elevators (Five-Year Requirements) aka Full-Load Tests: CAR SAFETIES 1002.3a - Inspect guide clearance, determine all working parts of the car safeties are in satisfactory operating condition, hand test to see safeties operate simultaneously with equal travel. Place rated load in car, center on each quarter of the platform symmetrically with relation to the centerline of the platform. Run elevator at rated speed, hand trip governor and bring car to rest on safeties. (For CWT safety test shall be made with no load in car). Take exact measurements to see that stopping distance conforms to requirements of the American Standard Safety Code for Elevators. Return safety to normal running condition. Re-inspect and check guide clearances. 7. Schedule: The contractor shall perform the tests listed in Section 6 as listed in the schedule below: Base Year: Contractor to witness the Semi-Annual Elevator Inspections, Hydraulic Elevators (Annual Requirements) aka Pressure Tests, and Traction Elevators (Annual Requirements) aka No-Load Tests. Option Year 1: Contractor to witness the Semi-Annual Elevator Inspections, Hydraulic Elevators (Annual Requirements) aka Pressure Tests, and Traction Elevators (Annual Requirements) aka No-Load Tests. Option Year 2: Contractor to witness the Semi-Annual Elevator Inspections, Hydraulic Elevators (Annual Requirements) aka Pressure Tests, & Traction Elevators (Five-Year Requirements) aka Full-Load Tests. Testing shall be done in cooperation with Contractor that is awarded the elevator maintenance agreement for the Canandaigua VAMC. Option Year 3: Contractor to witness the Semi-Annual Elevator Inspections, Hydraulic Elevators (Annual Requirements) aka Pressure Tests, and Traction Elevators (Annual Requirements) aka No-Load Tests. Option Year 4: Contractor to witness the Semi-Annual Elevator Inspections, Hydraulic Elevators (Annual Requirements) aka Pressure Tests, and Traction Elevators (Annual Requirements) aka No-Load Tests.
