- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart� 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotess are being requested and a written solicitation will not be issued. 246-21-Q-0141 this solicitation is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-5. This procurement is full and open competition under NAICS code 336112. Contractor shall provide a firm, fixed-priced all-inclusive quote to provide Brand Name or Equal Products. The provision at 52.212-3, -Commercial Items is applicable. ������������������������������������������ SEE ATTACHED STATEMENT OF WORK Evaluation Criteria: The evaluation will be based on Lowest Priced Technically Acceptable.� Must meet all the requirements as stated in the Specifications section of the Statement of work to be considered technically acceptable. The Indian Health Service will not consider your quote for award if you fail to provide Technically Acceptable- The offeror�s response shall provide conclusive evidence or support their ability to provide the Mobile Medical Unit in accordance with the attached Statement of work.������������������������ Registration in the System for Award Management (SAM) database at https://www.sam.gov.�� Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
