- INTENT TO SOLE SOURCE This is a notice of intent to proceed with a sole source requirement above the simplified acquisition threshold. The contractor is Long-Term Care Institute, Inc., The Madison, WI. This procurement is for the acquisition of Virtual Surveys at VA Community Living Centers (CLCs) for the Department of Veteran Affairs Health Administration, Office of Geriatric and Extended Care (GEC). The virtual survey process shall utilize the Centers for Medicare and Medicaid Services (CMS) process and be performed for a total of 15 facilities: 13 CLCs and 2 Spinal Cord Injury/Disorders (SCI/D) Long Term Centers. The period of performance will be 3 months from the date of award. The CLCs offer an array of services including, but not limited to: short stay services such as hospice, respite, skilled nursing, and rehabilitation. long stay services such as dementia care care for veterans with spinal cord injury and disorders; and mental health recovery care for eligible veterans. The Government intends to negotiate with only one source as implemented by the Federal Acquisition Regulation (FAR) Subpart 6.302-1 Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. Market research has found no other vendor can provide services that meet the unique set of standards that VA facilities and programs are currently Virtual Surveying for. Vendors that believe they can meet these requirements in the future may respond to this notice. The response should contain a brief description of the vendor's capability. Please also include the socioeconomic status and size of your business (large or small) and a copy of your Capability Statement. This notice is NOT a request for competitive proposals. Any vendor who wishes to submit information for this requirement should email their information to the attention of Mr. Twillie L. Curry III at twillie.curry@va.gov by 1300 30 September 2021. Only email submissions will be accepted.
