- Page 3 of 3 Sources Sought Notice | Wheelchair (Ambulette) Transportation Service for VAMC Dayton | Seeking qualified Small Business prospective offerors with OMTB certification to determine set-aside type (SDVOSB, VOSB or Small Business) for the forthcoming solicitation The purpose of this sources sought notice is to seek responses from all qualified Small Business prospective offerors who hold a current Ohio Medical Transportation Board (OMTB) certificate to perform wheelchair (ambulette) transportation service in the state of Ohio to determine the set-aide type (SDVOSB, VOSB or Small Business) for the forthcoming solicitation summarized as follows: Short Title: Wheelchair Transportation (Ambulette) Service for VAACC Columbus NAICS-485991: Special Needs Transportation Purpose: The purpose of this sources sought notice is to determine what type of set-aside is appropriate for this acquisition (SDVOSB, VOSB or Small Business). Place of Performance: Department of Veterans Affairs, Chalmers P. Wylie VA Ambulatory Care Center (VAMC), 4100 W. Third Street, Dayton, Ohio 45428 Anticipated Period of Performance: 1 November 2021 to 31 October 2026 comprised of a base year and four option years. General Requirements: The contractor shall provide all vehicles, personnel, management, transportation, materials, supplies, and equipment necessary to perform contract services as defined and specified in the PWS which shall be provided in the forthcoming solicitation. The contractor shall perform services under this contract in strict adherence to all current laws and regulations applicable to the state of Ohio unless supplemented to exceed those requirements as specified in the PWS. All certification, equipment, staffing and recordkeeping should be in compliance with the rules of the Ohio Medical Transportation Board and appropriate parts of Ohio Administrative Code for this service. Anticipated Requirements: The following are the anticipated not-to-exceed (NTE) quantities for this requirement per year: Wheelchair Transport | BASE RATE | One-Way Trip INSIDE Franklin County: 6,800 trips (one-way) estimated per year Wheelchair Transport | MILEAGE | OUTSIDE Franklin County: 47,000 miles estimated per year Wheelchair Transport | WAIT TIME | 15-minute Increments: 36 wait-time increments (15-minute increments) estimated per year Attachments for Reference: A draft version of the Performance Work Statement (PWS) is attached for your reference as appropriate. Request all Small Business prospective offerors interested in this requirement email their company name, DUNS number, CAGE code, small business socio-economic size status and a signed copy of their current OMTB certification in Adobe PDF format to perform wheelchair (ambulette) transportation service for the anticipated subject requirement to robert.bolcavage@va.gov not later than 1 June 2021. Only those prospective small business prospective offerors who are certified under NAICS-485991and submit a current signed copy of their OMTB certification in response to this sources sought notice will be considered responsive for the purposes of this FBO posting.
