Description: This request is for Hemodialysis supplies that will be used in the Hemodialysis Clinic at the VA Detroit. The supplies being requested are Hemodialysis Prep Pump Infusion Kit ref # 03-2622-3 430 Cases, High Dialyzer Filters Ref # 0500318E 500 cases, and Hemodialysis Opti flux Dialyzer Ref # 0500325E 290 cases. The delivery schedule would be 60 cases of every 2 months, first delivery on Dec 21, 2021 or as requested in a timely manner by the Inventory Manager per space availability.
