Description
SERVICES TO BE PROVIDED: The Contractor shall provide to the Department of Veterans Affairs, Buffalo VA Medical Center (VAMC), all the necessary equipment, supplies, labor, and a qualified technologist to enable the clinicians of the Urology and ENT Departments to perform procedures prescribed in the Statement of Work involving Low Watt & High Watt Holmium Laser services, Lithotripsy (ESWL) services, and C02 Laser services. The Contractor shall be responsible for and subject to all terms and conditions related to documentation and maintenance of all equipment, as prescribed by the Buffalo VA Medical Center and industry standards. All equipment used under this contract shall conform to electrical specifications provided by the Buffalo VA Medical Center and must be authorized for use in the United States by the U.S. Food & Drug Administration. The Contractor shall be responsible for providing a competent and trained technician to operate the equipment and shall provide documentation of training and certifications. The Contracting Officer s Representative, in coordination with the Urology Department and the Contracting Officer, will retain the right to deny proposed key personnel and refuse services by technicians that do not demonstrate a high level of competency. The Contractor shall conform to all VHA Directives, security requirements, and trainings required by the Buffalo VA Medical Center. As part of the services, the Contractor agrees to take commercially reasonable actions to provide the services to the Buffalo VA Medical Center at a technological level that will enable Buffalo VA Medical Center to take advantage of technological advancement in the medical laser industry, within the existing overarching Mobile Surgical Services technology field. EQUIPMENT AND SUPPLIES: Holmium Lasers Low Watt & High Watt Laser Systems and protective eyewear/gear meeting OSHA requirements. Supply of Micron Laser Fibers in the following sizes: 1000 550 365 200 At minimum 1 Certified Technician The Holmium Laser provided by the Contractor must be able to achieve a minimum of 80 watts of laser energy in order to meet the needs of the Buffalo VAMC. All lasers must run on 208 Volt/30 Amp circuits without the use of a converter or adaptor. CO2 Laser System CO2 Laser System and protective eyewear/gear meeting OSHA requirements. Supply of disposables/ Hand pieces for each case in the following sizes Sterile Fibers High Performance ENT 180cm length Standard ENT 180cm length At minimum 1 Certified Technician. Lithotripsy System Lithotripsy System and protective eyewear/gear meeting OSHA requirements. System will include but is not limited to: Lithotripter X-ray C-Arm Treatment Table Workstation Display Monitor Any other necessary equipment At minimum 1 Certified Technician. TECHNICIANS: Contractor Technicians shall be responsible for the following on all four (4) types of laser units: Deliver the equipment to Biomedical Engineering when arriving on-site so it can be inspected and inventoried prior to being placed in service in the OR. The vendor must provide Preventative Maintenance and Calibration Documentation at the time of this initial inspection. Shall wear Contractor Badge/ID issued by VA Police or check in upon arrival with the VA Police, then with Clinical Engineering (always pre-procedure) and then the COR. Setting up the unit. Ensure proper handling of delicate lasers and equipment before, during and after procedures. Test fire all equipment and inspect supplies prior to leaving Quality Assurance. Provide staff with necessary disposables needed in conjunction with the equipment. Maintain operational readiness of lasers, instruments, equipment and supplies at all times. Demonstrates competence in the care of all laser instruments, disposables, and equipment that is to include the correct handling/cleaning of instruments pre/post procedure. Knowledge and correct use of electrical voltmeter. Cleans and disinfects equipment and treatment rooms prior to and after each laser procedure. Regularly checks equipment to maintain it is in proper working condition and promptly reports any problems to management. Fully understands and adheres to hospital emergency, Laser safety, radiation safety, infection control and other related procedures. Maintains an accurate inventory of equipment in possession, as required for services needed. Follow instructions and assist the credentialed physician in the operation of the unit. Completes and provides a Laser Use Record for each case that utilizes a Laser
