Items to be quoted or its Equivalent per the attached RFQ BG404241 and its attachments: (1ea)��� Upgrade of MP6 Control console and general equipment needed for computerization of MP6 accelerator system 1 computerization of accelerator stabilization (TPS) system of MP6 accelerator system 1 Computerization of injector system (ground potential) of MP6 accelerator system 1 Computerization of 200kV injection deck on MP6 accelerator system 1 Computerization of dual ion sources and vaccum stations on MP6 accelerator system 1 Installation of above items by two(2) experienced NEC engineers/technicians for two weeks. No spare parts included. System must be fully functional at time of installation.��� (1ea)��� Upgrade of MP7 Control console and general equipment needed for computerization of MP7 accelerator system 1 computerization of accelerator stabilization (TPS) system of MP7 accelerator system 1 Computerization of injector system (ground potential) of MP7 accelerator system 1 Computerization of 200kV injection deck on MP7 accelerator system 1 Computerization of ion source and vaccum stations on MP7 accelerator system 1 Installation of above items by two(2) experienced NEC engineers/technicians for two weeks. Please also return the following along with your quote (See attachments): 1.���������AMS-Form-009: Representations and Certifications � Procurement Specific.� Due to the value of this contract, DOE requires Business Representations and Certifications be completed for all procurements exceeding $10,000.� Please reference Solicitation Number 404241 on the form and all correspondence.� Please complete and return all twelve (12) pages of the document. 2.���������Published Price list: showing the items on your quote for price justification or a web screenshot (or link) of same/similar items with pricing and part number shown. 3.�������� Insurance: �**IF APPLICABLE** Upon award, in accordance with Article 6 of�the attached �BSA Supplemental Terms and Conditions for Work by Contractors On Site at Brookhaven National Laboratory�.� Prior to coming on site to perform work, the certificate must remain in effect for the duration of the subject contract and must be filed with BNL Procurement & Property Management Division. 4.���������System For Award Management (SAM): In order to support DOE reporting requirements, BSA requires all its vendors/contractors to be registered in the U. S. Government supplier database entitled System For Award Management (SAM) with their Dun & Bradstreet number (Duns#). Registration can be performed on line at www.sam.gov. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization. �Please note that there is no charge for SAM registration. When registering, you are strongly urged to copy and paste www.sam.gov onto your browser address rather than utilizing your search engine. Utilizing your search engine may redirect you to an erroneous site. You may obtain assistance in completing your registration by contacting the Federal Help Desk� at 866-606-8220. 5.����������Equivalent Guidelines: The parts or model numbers are listed for reference purposes only. Any alternate products being offered shall have equivalent, or greater, performance parameters to those specified. Suppliers that are offering alternate products shall list the manufacturer, model, and part numbers for the product. When quoting an alternate product, please provide sufficient documentation to permit thorough evaluation of the equivalency of your offer to the product specified.� BSA reserves the right to determine equivalency. 6.���������Basis of Award: BSA will make award to the responsive and responsible offeror who submits the lowest total price and who meets all of BSA�s technical requirements, as stated herein. 7.�������� Pricing: shall remain valid for 60 days Quotations will be opened in private. Quotations, including their withdrawal or modification, received after the closing date shall be considered late. Such responses will only be considered for award if it is in BSA�s best interest to do so. Any resulting purchase order will be subject to the attached terms and conditions.
