- Waste Disposal Service Statement of Work Scope of Work: The Statement of Work is for performance of work associated the tracking and disposal of waste and recyclables. The contractor shall provide Kansas City VA Medical Center with the proper skilled labor, equipment, materials, supplies, safety, and supervision for performance of all work indicated or referenced within this Statement of Work and by the corresponding Construction Documents (plans, specifications, all VA TIL publications, or other provided documents). Description of Work: PART I. GENERAL REQUIRMENTS The contractor shall provide one 40 cubic yard waste container for construction debris and one 20 cubic yard dumpster for recycled materials for the Kansas City VA Medical Center located at 4801 Linwood Blvd., Kansas City, MO 64128. PART II. SPECIFICATION The 40 cubic yard container must be picked up daily during the work week(M-F) for the first 90 calendar days. The 20 cubic yard container must be picked up Monday, Wednesday, Friday for the first 90 calendar days. Both dumpsters will be call in after the first 90 days. Location of the dumpster on the VA Medical Center campus is to be determined (TBD). Background: The in-house construction crew will be renovating the Emergency Department of the main building. 1. Services to be provided by the Contractor: a). Contractor shall provide all waste containers. Contractor provided containers shall be of commercial quality used regularly at other similar construction activities in the community. Quality and type of contractor provided containers will be subject to the approval of the Contracting Officer. b). Contractor shall provide all transportation of refuse form the VA Medical Center to approved landfills. Contractor shall maintain licenses and/or permits required by federal, state, and local laws/regulations. c). Contractor provided container shall come in either 20 or 40 cubic yard sizes unless agreed upon by the COR and Contractor and approved by the Contracting Officer. d). All contractor provided containers shall be cleaned and sanitized prior to placement at the VAMC. The contractor shall immediately exchange contractor provided containers at no cost to the VAMC due to containers being placed into service which are dirty or foul smelling. e). Contractor shall provide additional dumps of containers upon request of the Contracting Officer or COR. The cost per additional dump shall be the same as the regularly scheduled dumps. Response time shall be within 8 hours after notification. f). All residual waste from container spillage caused by the contractor shall be picked up by the contractor prior to leaving the area. The area shall be left broom clean . g). Broken or damaged containers shall be replaced within two (2) hours of notification. 2. Other Terms and Conditions a). Pick-up Hours: All container dumps shall be accomplished during the hours of 6:00 AM to 4:30 PM (holidays not included); however, special dumps may be requested by the VA on any day of the week at any hour. b). Replacement containers: Contractor must furnish empty containers to be left at the time of each dump for both containers. c). Contractor shall provide report of each container containing dates when dumped and weights for each dumpster with the monthly invoices. d). The contractor shall credit the VAMC with all proceeds obtained from Recycling Center(s) within 45 days of dump. The proceeds shall also be reflected in the original statement(s) from the Recycling Center(s), which must include items recycled, date of dump, weight of dump and the selling price of the item. e). Contractor must be prepared to commence work no later than 15 calendar days after notification from COR. f). Billing periods will be monthly. Contractor shall submit a monthly itemized invoice in arrears. Invoice shall clearly show the dates of service, the containers dumped, and any credit received for recycled products. Invoices must be submitted to http://www.fsc.va.gov/einvoice.asp PART III: GENERAL ADMINISTRATION a. Contract Officer Representative (COR) Contact: the COR to coordinate this service. The contractor will contact the COR to schedule the work to be completed; upon arrival, the contractor will contact the COR. The COR will escort the contractor to the appropriate area and monitor the work being done. Upon completion of the work, the COR will escort the contractor out of the area. b. Place of Performance: Kansas City VA Medical Center 4801 Linwood Blvd Kansas City, MO, 64128 c. Period of Performance: This contract will be for a base year and include 4 one-year option periods
