Sources Sought Notice Sources Sought Notice Page 5 of 5 Sources Sought Notice 36C25621Q1645 Sources Sought Notice Page 1 of 5 SOURCES SOUGHT NOTICE Community Nursing Home Services Overton Brooks VA Health Care System Shreveport, LA DESCRIPTION: This is a sources sought notice/request for information is issued in order to determine the availability of potential sources having the skills and capabilities necessary to provide the appropriate level of Community Nursing Home (CNH) care to the beneficiaries the Department of Veterans Affairs, Overton Brooks VA Healthcare System (OBVAHS) located at 510 E. Stoner Ave. Shreveport, LA 71101. OBVAHS has a requirement for Community Nursing Home Services within its catchment area of Shreveport, Bossier City, Monroe, LA to include the surrounding parishes of Caldwell, Franklin-Madison, Jackson, Ouachita, Union, Moorehouse, West Carroll, East Carrol, Richland, Tensas to Caddo, Bossier, Webster, Claiborne, Lincoln, Bienville, and the counties of Red River, Sabine and Desoto, Texas. The estimated period of performance will be for a period of 5 years from the date of award of the contract(s). The number of veterans requiring Community Nursing Home services (bed days) over the next five years will fluctuate and cannot be adequately determined at this time. Therefore, the award of multiple Fixed Unit - Price Indefinite Delivery Contracts (IDC) is anticipated throughout the catchment area. Please be aware all Federal, State and local labor laws will be applicable to the pending procurement including, federal minimum wage as provided in Section 6(a)(1) of the Fair Labor Standards Act. If you have specific questions regarding labor laws, you should contact your local Federal Department of Labor representative. In addition, the solicitation will include the class deviation in accordance with Federal Acquisition Regulation (FAR) 1.404, to implement Executive Order (E.O) 14042. FAR 52.223-99, Ensuring Adequate COVID Safety Protocols for Federal Contractors will be included in the contract in order to maximize the goal of getting more people vaccinated and decrease the spread of COVID-19. REQUIRED: Interested agencies will be Medicaid certified to their location as a Certified Nursing Home Facility and licensed as a Nursing Home by the State of Louisiana or Texas (as applicable) in which its provider number is based. All interested vendors are invited to provide information to contribute to this market survey/sources sought including commercial market information. THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a source sought notice. Questions and response to this notice should be submitted by email to toni.clendenen@va.gov. Provide only the requested information below. The purpose of this notice is to gain knowledge, in this order, of potential qualified sources and their size classifications (Service Disabled/Veteran Owned Small Business (SDVOSB/VOSB), Hub zone, 8(a), small, small disadvantaged, woman owned small business, FSS/GSA contract schedule holders or large business) relative to NAICS 623110 Nursing Care Facilities (size standard $30.0 Mil). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this sources-sought synopsis are not considered adequate responses to the potential solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this source sought announcement. REQUESTED INFORMATION: (1) STATEMENT OF CABABILITY: Submit a brief description, five (5) pages or less, that demonstrates how your company can provide the requested services at time of award. Include your company s experience in performing these services to the VA, other Government (Federal or State) agencies, or for a private facility. (2) BUSINESS SIZE AND SOCIO-ECONOMIC STATUS: (a) Indicate whether your business is large or small as defined by the NAICS (b) If small, indicate if your firm qualifies as a small, emerging business, or small disadvantaged business (c) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act (d) Indicate if your firm is a certified Hub-zone firm (e) Indicate if your firm is a woman-owned or operated business (f) Indicate if your firm is a certified Service-Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB). (g) Include the DUNS number of your firm. In accordance with FAR 52.204-7, System for Award Management, prospective contractors must be registered in the System for Award Management (SAM) and pursuant to Public Law 109-461 (38 U.S.C. 8127 and 8128), Implementation Policy and Procedures be registered in VetBiz and certified as an SDVOSB/VOSB by the Department of Veterans Affairs Office of Small and Disadvantaged Business Utilization (OSDBU) to be eligible for award. Contractors may obtain SAM information at https://beta.sam.gov and Vet Biz information at https://www.vip.vetbiz.gov. There is no cost for registration and potential offerors are encouraged to visit both websites and register well in advance. No contracts can be awarded to any offeror not registered in SAM nor will self-certification as an SDVOSB/VOSB be acceptable. Responses must be emailed to toni.clendenen@va.gov no later than 3:00PM (CST), October 18, 2021. Your response should include both the STATEMENT OF CABABILITY and BUSINESS SIZE AND SOCIO- ECONOMIC STATUS information as explained above.
