Description
NOTICE OF INTENT TO SOLICIT FROM A SINGLE SOURCE VA VISN 2 s intends to negotiate with a single source in accordance with FAR 13.106-1(b)(1) to obtain a full service maintenance agreement for VA owned Tablo Hemodialysis systems. The source identified below has been identified as the sole source for this service. All responsible sources who believe they can furnish the service described in this notice may submit a capability statement, proposal, or quotation, which shall be considered by the agency by emailing Joseph.Cellino@va.gov and Leia.Labruna@va.gov with details regarding your firm s capability no later than 4:00PM EST on October 20, 2021. Responses must provide sufficient detail to establish capability. PERIOD OF PERFORMANCE: Approximately 11-01-2021 to 10-31-2022, with four subsequent option years IDENTIFIED SOURCE: SDV Office Systems, LLC DBA: SDV Medical DUNS: 964786078 26 Macallan Ln Asheville, NC 28805 VISN 2 FACILITIES TO BE INCLUDED: VA NYHHS - Brooklyn Campus 800 Poly Place Brooklyn, NY 11209 VA NYHHS - Manhattan Campus 423 E 23rd St New York, NY 10010 Northport VA Medical Center 79 Middleville Road Northport, NY 11768 East Orange VA Medical Center 385 Tremont Avenue East Orange, NJ 07018 Syracuse VA Medical Center 800 Irving Ave Syracuse, NY 13210 Stratton VA Medical Center 113 Holland Ave Albany, NY 12208 BRIEF DESCRIPTION OF REQUIREMENT: A Full Service contract is to include all necessary labor, full service, maintenance, tools, equipment, transportation and OEM parts required to perform repair and Preventive Maintenance (PM) on listed equipment. All equipment shall be maintained in proper operating condition as specified by the manufacturer. Vendor must have manufacturer certification to service and maintain the devices and have access to Original Equipment Manufacturers (OEM) parts, manuals and schematics to perform the service. Vendors must have their own schematics available on site at time of service. Scope of Work: Contractor is required to furnish all original equipment manufacturer (OEM) parts and labor to provide maintenance of the Tablo devices, in accordance with OEM guidelines. Contractor must have manufacturer certification to service and maintain the above equipment. Contractor will provide a maximum of 8 carbon filters and 4 sediment filters per device per annum. Additional filters required will be required to be purchased by the VA facility outside of this agreement. The VA facility is responsible for replacement of the carbon and sediment filters. Contractor shall test the functionality of the equipment upon completion of repair. If this is a test for repair, the Test should ensure that damaged parts have been successfully repaired and all functions including, but not limited to those related to repaired parts, are operational. If this is a test for a routine PM service, inspection, etc., then the test should just ensure that the equipment is functioning within the guidelines of the OEM or the applicable standards (OSHA, ASSE, JCAHO, NEFP, etc). Repair services at the request of the VA, including support for recalls issued by NCPS/FDA, will be provided to diagnose and correct equipment malfunctions on a routine basis. All repairs shall be performed by a manufacturer-trained technician. Contractor will follow manufacturer's recommendations for PM, repair, calibration and modifications Preventive maintenance: PMs will be scheduled in advance and conducted ANNUALLY. It is the contractor's responsibility to schedule preventive maintenance as listed. PM procedures subscribed to must follow manufacturer s specifications and must be documented and submitted in writing to biomedical engineering. Upon completion of PM procedures, an applicable label should be applied to the unit, indicating the date of PM performance, the next date following PM is due, as well as the initials of the individual that performed the PM. At inception of contract, PM shall include, at a minimum, electrical safety testing, lubrication adjustment, calibration, testing and replacement of faulty parts and/or parts which are likely to fail at no additional charge. Software Updates: Contractor will update the devices with software updates to the licensed software as they are released. Any external hard drives our USB flash drives must be submitted to biomed for a cybersecurity scan prior to use on VA owned equipment.
