- VISN 19 - Custom Sterile Surgical Packs / Custom Procedure Trays (CPTs) The purpose of Request for Proposal (RFP) 36C25919R0099, Amendment 0003 (effective Oct. 18th, 2021) is to; a. Extend the Proposal Submission Date shown on Pg. 1 (SF1449) and Pg. 46 (Instructions to Offerors) to provide the Government additional time to address access to RFP attachments and respond with answers to the questions submitted by the RFP questions cut-off date. b. The Proposal Submission Date is hereby changed from October 21st, 2021, 4:00pm (EST) to November 4th, 2021, 4:00pm (EST).
