STATEMENT OF WORK Solicitation # 36C24522Q0048 GENERAL INFORMATION Title of Project: Bayer Contrast Injector Service Contract, Per Scope of Work: The vendor shall provide maintenance, support, and repair services on all equipment listed below to include preventive maintenance and calibration in accordance with the original equipment manufacturer (OEM) written specifications. The vendor shall provide travel, parts, and labor to complete all services covered under this agreement. All parts must be certified to the OEM specifications. The vendor shall provide necessary hardware enhancements to upgrade workstations for each device into Certegra Workstation 3.0 and to upgrade contrast injectors for each device into MEDRAD Stellant FLEX models. The vendor shall provide appropriate software updates to operate each device with these hardware enhancements. The vendor will supply the VA Maryland Health Care System Biomedical Engineering department with a copy of a detailed service report within 2 weeks of maintenance being completed. The service shall be performed during normal business hours Monday through Friday from 8:00am to 4:30 pm unless mutually agreed upon by the contractor and the VA Maryland Health Care System Biomedical Engineering staff. Contractors shall report to Biomedical Engineering in room 5B183 in Baltimore when they arrive on-site before performing any maintenance, scheduled or unscheduled. Equipment Serial # Bayer Medrad Stellant D CT Injector 31473 Bayer Medrad Stellant D CT Injector 204061 Bayer Medrad Stellant D CT Injector 40477 Bayer Medrad Stellant D CT Injector 34128 Background: The VA Maryland Health Care System requires that all imaging equipment be maintained in accordance with the manufacturer s specifications. This equipment will be covered under a support agreement when it represents the best value for the Government. Performance Period: 1 year beginning at time of award plus two options. Type of Contract: Maintenance services Place of Performance: Baltimore VA Medical Center 10 N. Greene St. Baltimore, MD 21201 Performance Based Contract: N/A Security: The contractor must have photo ID available when performing work at a VA Medical Center and be willing to present it to VA staff. The vendor is responsible for personal protective equipment. The vendor must have a current Business Associate Agreement (BAA) with the Department of Veterans Affairs to perform maintenance on equipment that would result in incidental access to patient Protected Health Information (PHI).
