Description
The purpose of this amendment is to correct the date/time of closing and to provide changes to the content of this document. The Department of Veterans Affairs, Network Contracting Office 09, hereby provides notice of its intent to award a sole source, firm-fixed price contract for sterile storage cabinets to Aldevra, LLC, 3707 S. Westnedge Ave., Kalamazoo, MI 49008. The contract is expected to be awarded under the authority of 41 U.S.C. 3304(a)(1), as implemented by FAR 13.106-1(b) Soliciting from a Single Source. A Justification for Single Source Awards for Other Than Full and Open Competition has been prepared. The anticipated award date is November 1, 2021. NAICS code: 339113 Surgical Appliance and Supplies Manufacturing (SBA Size Standard 750 Employees) PSC: 6515 - Medical and Surgical Instruments, Equipment, and Supplies THIS NOTICE OF INTENT IS NOT A SOLICITATION OR REQUEST FOR COMPETITIVE QUOTES. However, all responsible sources may submit an offer and technical capabilities/narrative that is equal to or exceeds the technical standards set by this requirement. A determination not to compete this requirement, based upon responses to this notice, is solely within the discretion of the Government. Information received will normally be considered solely for determining whether to conduct a competitive procurement. The Department of Veterans Affairs will NOT be responsible for any costs incurred by interested parties in responding to this notice of intent. Only written responses will be considered. All responses from responsible sources will be fully considered. As a result of analyzing responses to this notice of intent, the Government shall determine if a solicitation will be issued. Any prospective contractor must be registered in the System for Award Management (SAM) to be eligible for award and must submit a current authorized distributor letter from the manufacturer. All Service-Disabled Veteran-Owned and Veteran-Owned Small Businesses and other socioeconomic designated small businesses must submit a self-certifying statement that their company meets all criteria established IAW 13 CFR 121.406(b)(1)(i) through (b)(1)(iii) thus demonstrating compliance with the Non-Manufacturer Rule. The capability documentation shall include general information and technical background describing in detail the prospective contractor's capabilities and experience that will demonstrate the ability to support these requirements Any interested vendor needs to provide an authorized original equipment manufacturer (OEM) distributor/dealer/reseller letter (if not a manufacturer) and answer the following questions: How many employees do you have? Do you primarily engage in the retail or wholesale trade, normally selling the type of item sought it in this requirement? Do you take ownership or possession of those normally procured items through your personnel, equipment, or facilities until you sell or deliver them? Are the items you selling produced by other small businesses? If you are doing the drop-shop method, are you paying the freight/shipping from the manufacturer/supplier to the customer? Are you authorized and trained to install, troubleshoot, repair, or generally work on the system? Do you offer all the products and services under existing Federal Government contracting vehicles? if yes, please provide the contract number/identifier. Does the product you are offering have to be modified to meet this requirement? If so, provide details on the extent of modification and how long it would take to modify. REQUIREMENT: Quantity: 2, Air Innovations SCC 4500 Sterile storage cabinets with installation and training services. Salient Characteristics Overall unit dimensions: 30.5"" Wide x 32"" Deep x 77.5"" High 520 lbs. 5 shelves 27 3/4"" Wide x 18"" Deep Overall Payload Dimensions: 28"" Wide x 19"" Deep x 42"" High VA Standards: VA Directive 1116(2); CSA (Canadian Standards Association) Z314.15-10; ANSI/AAMI ST79-2006; ANSI/ASHRAE/ASHE Standard 170-2017 The SPS Preparation Area requires temperature 66-75ºF, humidity 30-55%, positive filtration and ten air exchange parameters. The SPS Decontamination Area requires temperature 66-72ºF, humidity 30% to 55%, negative filtration, and ten air exchange parameters. Storage areas for critical and semi-critical RME require temperature 66-75ºF, humidity 30-55% with positive filtration and four air exchange parameters in order to protect RME integrity. A monitoring system capable of documenting continuous humidity and temperatures is required in all SPS areas. SPACE REQUIREMENTS Building environmental conditions 60 - 82°F, 60 F Dew Point Demineralized water 120-1-60 supply, grounded 15 AMP (10 RLA) room receptacle Minimum recommended room size 10 x 10 . For installations of 3 or more units in one room please contact factory for assistance. Statement of Work Scope of Work: Prior to installation date, SPS will: Conduct site preparation with vendor for need, electrical and filtration requirements for Newburg CBOC Women s Clinic, located at 3430 Newburg Road, Room C214, Louisville, KY 40218-3483 Establish location for shipping and handling of both cabinets and schedule delivery to Newburg Clinic Will provide SPS leadership representation along with vendor representation to ensure cabinets are delivered safely and work accordingly. During Installation, vendor will: Conduct complete inventory of all items that have been delivered to VA hospital for installation Confirm all supplies have been obtained for proper installation Complete Installation Checklist Conduct tests on all cycles, searching for possible malfunctions. After installation is complete, Vendor Sales Representative will: Conduct In-service / Overview Completes training and signs training sheet Operation of the Machine Safe use of Machine Chemical Handling Emergency Procedures Regular Maintenance Procedures Conducts additional in-service training for employees absent during initial training Review manufacture IFUs with all employees Review Troubleshooting and most common mistakes made Delivery / Installation Schedule: Items will be delivered and installed at Newburg CBOC Women s Clinic, 3430 Newburg Road, Room C214, Louisville, KY 40218-3483 during the weekday(s) Monday through Friday after patient hours from 8:00am to 4:00pm.to avoid interruption to regular operations. Hours and Date of installation will be negotiated between shareholder and vendor. Project Completion Will be Determined Successful When: All equipment is installed and is operational during in-service Vendor Representative has adequately trained to the stakeholder s expectations Warranty: Free from defects in material and workmanship for one year from the date of installation.
