- Page 2 of 2 This Sources Sought Notice is for informational and planning purposes only and shall not be construed as an obligation or commitment by the Government. This notice is intended strictly for market research. The purpose of this Sources Sought notice is to obtain information from qualified vendors with special interest relative to the North American Industry Classification Code (NAICS) 325998 - All Other Miscellaneous Chemical Product and Preparation Manufacturing. The Government will not pay for information received in response to this Sources Sought, nor will the Government compensate any respondent for any cost incurred in developing the information provided to the Government. This Sources Sought does not constitute a commitment by the Government. Information provided in response to this market survey will be used for informational purposes only and will not be released. Vendor participation is not a promise of future business with the Government. Please respond to this sources sought notice if you wish to be considered for this procurement. All responses must demonstrate how the commodity meets or exceeds the salient characteristics provided below. STATEMENT OF WORK GENERAL DESCRIPTION OF SUPPLY ITEM Contractor shall provide the following road salt for the Clement J. Zablocki VA Medical Center, 5000 W. National Avenue, Milwaukee WI 53295. Vendor must call 414-384-2000, ext. 41061, and speak to Cherie Ladwig to schedule date and time of delivery. Contractor shall deliver three 200 Ton shipments, the first not later than (NLT) 01/1/2022, the second NLT 03/01/2022, and the third NLT 05/01/2022. Offeror shall provide a total of 600 tons of road salt. SALIENT CHARACTERICS: A. Road salt to be free of sand, stone, fillers, debris, and must not be water saturated, frozen, or clumped. Upon inspection, if road salt is found to be contaminated with any kind of debris, the shipment/delivery will be halted. B. Road salt material effectiveness/composition cannot be altered by moisture. C. Weight tickets must be provided upon delivery. Loads of salt will not be accepted unless a delivery ticket is provided upon arrival. D. Delivery requires use of a truck with open trailer to unload salt (dump truck). E. The government reserves the right to change the storage location and/or the amount of salt delivered. F. The contractor is responsible for immediately cleaning up any spills, debris, etc., which may occur during delivery. G. Safety Data Sheets (SDS) shall be provided. H. Contractor shall be able to deliver within 48 hours after call-in request. No limit on the amount delivered. I. No plain rock salt. No liquid de-icer. Must be no more than 12 millimeters (mm). J. Must be effective at -20 to -25 degrees Fahrenheit. 1. Delivery Schedule: Ship to: Clement J. Zablocki VAMC 5000 W. National Ave. Milwaukee, WI 53295 Coordinate with VAMC POC. 3.0 PRODUCT MODIFICATION, REMOVAL, OR RECALL: If any product awarded under this contract requires modification, is removed or recalled by the Contractor or manufacturer, or if any required modification, removal, or recall is suggested or mandated by a regulatory or official agency, the following steps will immediately be taken by the Contractor or manufacturer: A. Notify the Contracting Officer, in writing, by the most expeditious manner possible. Provide two copies of the notification which shall include, but not be limited to the following: 1. Complete item description and/or identification, order numbers from customers and the contract number assigned. 2. Reasons for modifications, removal, or recall 3. Necessary instructions for return for credit B. Provide the above information to all agencies and VA Facilities who purchased the product. The Contracting Officer shall be provided a copy of the notification in b. above, and a list of all agencies and/or VA facilities notified C. Contractor shall be responsible for any costs associated with the above. 4.0 HOURS OF OPERATION: Normal hours of operation are Monday through Friday from 7:00 am to 3:30 pm, excluding holidays. Federal and National Holidays observed by the VAMC are: National Holidays Federal Holidays New Year's Day Labor Day Independence Day Memorial Day Thanksgiving Day Christmas Day M L King's Birthday Columbus Day President's Day Veterans Day Juneteenth National Independence Day 4th of July Independence Day 1. Also included would be any other day specifically declared by the President of the United States to be a National Holiday. 2. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. 5.0 SECURITY REQUIREMENTS The Contractor s employees shall wear visible identification and company uniforms, approved by the POC. 6.0 DELIVERY CHECK-IN REQUIREMENTS The Contractor shall report to the VAMC POC. Upon completion of each service visit, the Contractor shall provide a weight ticket. Due to the recent pandemic, contractors must be able to meet the screening policy in effect at the Clement J. Zablocki VAMC. Contractor Personnel who screen positive, will not be allowed to enter any building on campus. Contractor Personnel must be able to answer NO to all of the following questions: 1. Do you have a fever? 2. Do you have a sore throat? 3. Do you have a new shortness of breath, within the last 21days? 4. Do you have a new onset of a cough in the last 21 days? 5. Have you traveled outside the U.S. in the last 14 days? B. The Occupational Safety and Health Administration (OSHA) has issued an Emergency Temporary Standard for Healthcare Workers and Contractors. Healthcare Workers and Contractors are required to wear the approved Food and Drug Administration (FDA), facility issued, face mask. Cloth masks, gaiters and scarves are not acceptable forms of protection. The FDA approved mask must be worn over their nose and mouth when working on campus within six feet of another individual when outside, and at all times when inside a building. 8.0 GENERAL INFORMATION Smoking not be permitted anywhere on campus by contractor personnel nor by VAMC employees. Upon request, a copy of hospital smoking policy will be supplied. Violation of the VAMC Milwaukee smoking policy may result a person to be removed immediately from the campus grounds and from providing the service and/or goods. Interested vendors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract.
