- Opened up to Small business as a Total Small Business Set aside. The Indian Health Service Lockport Service Unit has a requirement for a contactor to provide�quarterly deep cleaning of the Lockport service unit and�backup janitorial services when Lockport�s Custodial worker is off on holiday, or when they call in sick. 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 2. The Solicitation number of this Request for Quote (RFQ) is 285-21-RFQ-0026 3. The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-04, dated January 25, 2021. 4. This is a combined synopsis/solicitation is 100% set aside for Small Business. The associated North American Industry Classification System code (NAICS) code is 561720, Janitorial Services, and the size standard is $19.5�million. 5. Products and services to be provided by the contactor Contractor shall serve as a�backup janitorial services when Lockport�s Custodial worker is off on holiday, or when they call in sick.�Contractor must be able to arrive with the same day notice. Contractor will also provide Lockport with a quarterly deep cleaning of the facility. Contractor will provide the supplies for the deep cleaning service. Period of performance� Base Year:� � ���Award -� September 30, 2021 Option Year 1: November 1, 2021 - September 30, 2022 Option Year 2: November 1, 2022 - September 30, 2023 Option Year 3: November 1, 2023�- September 30, 2024 Option Year 4: Novemberf 1, 2024 - September 30, 2025 6. Description of requirements Cleaning duties include but are not limited to Floors 1. All tile floors will be swept and mopped each day. 2. All carpeting and runner mats will be vacuumed each day. 3. Special attention will be paid to edges and corners of all tile and carpeted areas. *Shampooing when needed. Dusting 1. All tops of cleared desk, tables and file cabinets will be dusted each day. 2. All other horizontally flat surfaces such as mini blinds, window ledges, book shelves, etc will be dusted weekly. 3. All high dusting such as door sashes, ledges, and moldings will be dusted monthly. 4. Remove smudges, marks, etc. from walls and doors as needed. Restroom Care 1. All toilets and urinals will be cleaned and disinfected each day. 2. All sinks and mirrors will be washed each day. 3. Floors will be swept and mopped with a germicidal cleaner each day. 4. Stalls, walls and baseboards will be spot cleaned each day and washed monthly. 5. Soap dispensers will be filled as required. 6. Toilet tissue and paper towel dispensers will be filled as required. Exam and Operatories & Lab Rooms 1. All exam, Operatories and lab floors will be swept and damp mopped�(Dental will only be swept and mopped) 2. All sink and countertops will be wiped down with a disinfectant cleaner. 3. All equipment and chairs will be wiped down each day with a disinfectant cleaner. 5. Soap dispensers will be filled as required. 6. Paper towel dispensers will be filled as required. Glass 1. Entrance glass doors will be cleaned inside and outside each day. 2. All interior glass and partitions will be spot cleaned as needed. Kitchen Area 1. Floors will be swept and damp mopped each day. 2. All counters, tables and chairs will be wiped down and cleaned each day. 3. Sinks will be cleaned and sanitized each day. 4. Damp wipe interior of microwave each day. 5. Soap dispensers will be filled as required. 6. Paper towel dispensers will be filled as required. G. Waste Material 1. All waste material will be collected and placed in a dumpster each day. 2. All trash cans will be emptied each day and liners will be replaced as needed. Miscellaneous 1. Water fountains will be cleaned and disinfected. 2. All telephones will be cleaned and disinfected 3. Air conditioning/heater vents will be cleaned and dusted 4. Training room w/ Kitchen will be cleaned as needed. 5. Exercise room and equipment will be cleaned and wiped down as needed. Lobby and Waiting Rooms All equipment and chairs will be wiped down each day with a disinfectant cleaner. All High touch areas will be wiped down hourly with a disinfectant cleaner. (Door handles, front desk reception, knobs, light switches) 7. Performance shall be within 15 days of date of award. 8. The Contractor shall submit invoice after services received. Failure to submit directly to the offices listed above will delay prompt payment of your invoice. For invoices submitted by email: the subject line of your email invoice submission shall contain the contractor name, contract/order number, and invoice number (e.g. Company Name, Contract HHSIXXXXX, Invoice #123). The Contractor shall send one email per contract/order after completion. The email may have multiple invoices for the contract. Invoices must be in one of the following formats: PDF, TIFF, or Word. No Excel formats will be accepted. The electronic file cannot contain multiple invoices; example, 10 invoices requires 10 separate files (PDF, TIFF, or Word). 9. Addendum to FAR 52.212-1 Instructions to Offerors � Commercial Items: Offerors are to e-mail complete quotes to paul.carr@ihs.gov no later than date required by notice posted on GPE.
