Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.�IHS intends to award a minimum of one, maximum of two Blanket Purchase Awards in respose to this solicition. The Solicitation number of this Request for Quote (RFQ) is 285-21-RFQ-0028. The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06 June 10, 2021. This is a combined synopsis/solicitation is 100% set aside for small business enterprises, the NAICS code 423450, Medical, Dental, and Hopsital Equipment Supplies Merchant Wholesalers, and�the�size starndard is 200 emploees. Products and services to be provided by the contactor: This Scope of Work describes the requirement for purchase and delivery of durable medical equipment to be provided to the Lockport IHS Health Center, 150 Professional Parkway Lockport, NY 14094. �DME meets these criteria: i. Durable (can withstand repeated use) ii. Used for a medical reason iii. Not usually useful to someone who isn't sick or injured iv. Used in your home v. Generally has an expected lifetime of at least 3 years Medicare will only cover DME if the provider and DME suppliers are enrolled in Medicare. Doctors and suppliers have to meet strict standards to enroll and stay enrolled in Medicare. PRC is considered the Payer of Last Resort and cannot remit payment until all other alternate resources are exhausted. For eligible Medicare Part B recipients: The contractor will need to bill Medicare first before payment can be made by the individual PRC program. A denial or billed claim will need to be attached to the request for payment (if the beneficiary has Medicare coverage). The requested DME must be medically necessary to be considered under this contract. A physician�s request must be presented for the request of DME services. The contractor is fully responsible for verifying and validating all information contained within this scope. A list of available DME will be made accessible to the Service Unit (SU) for use in determining obtainable resources. A report for each encounter will be faxed or mailed to the designated Federal Service Unit within 7 days after the visit. The contractor will provide the appropriate Service Unit (SU) with any and all reports should additional services be medically necessary within 7 business days following initial appointment for medical staff at the SU to follow-up on. All work shall conform to, and comply with, the latest edition/version of ALL applicable federal, state, and local laws, rules, regulations, codes, and standards. �Medicare Part B (Medical Insurance) covers medically necessary DME if the medical provider prescribes it for use in your home. �DME that Medicare covers includes, but isn't limited to: i. Blood sugar meters ii. Blood sugar test strips iii. Canes iv. Commode chairs v. Continuous passive motion devices vi. Continuous Positive Airway Pressure (CPAP) devices vii. Crutches viii. Hospital beds ix. Home infusion services x. Infusion pumps & supplies xi. Lancet devices & lancets xii. Nebulizers & nebulizer medications xiii. Oxygen equipment & accessories xiv. Patient lifts xv. Pressure-reducing support surfaces xvi. Suction pumps xvii. Traction equipment xviii. Walkers xix. Wheelchairs & scooters The Contractor agrees to maintain as required by law accurate medical records, which sufficiently and properly reflect the SERVICES performed. Such records shall be faxed to the Lockport IHS Health Center 716-280-3868 and shall be subject to retention by Lockport in accordance with all federal, state, and local laws applicable to medical records Contractor shall provide and maintain malpractice and such other insurance during the period of this contract, at no additional cost to the Government. All insurance required by the contract shall be in a form and amount as Contracting Official may deem necessary as per the Federal Acquisition Regulation (FAR) 52.237-7 ""Indemnification and Medical Liability Insurance"" clause states. Proposal Instructions Pricing Shall be provided under CMS/DMEPOS Fee Pricing including percent discount Quote's should contain�a capabilities statement including all services�under the scope the contractor is able to provide.� Quotes should include the past 3 years of past perfromance to any Federal, State, Local entities Period of Performance Base Year - Award - September 30, 2022 Option Year 1 - October 1, 2022 � September 30, 2023 Option Year 2 - October 1, 2023 - September 30, 2024 Option Year 3 - October 1, 2024 - September 30, 2025 Option Year 4 - October 1, 2025 - September 30, 2026 The Contractor agrees to maintain as required by law accurate medical records, which sufficiently and properly reflect the SERVICES performed. Such medical records shall be and remain the property of the Lockport Service Unit and shall be subject to retention by the Lockport Service Unit in accordance with all federal, state, and local laws applicable to medical records. Contractor shall provide and maintain malpractice and such other insurance during the period of this contract, at no additional cost to the Government. All insurance required by the contract shall be in a form and amount as Contracting Official may deem necessary as per the Federal Acquisition Regulation (FAR) 52.237-7 �Indemnification and Medical Liability Insurance� clause states. Performance will take place at�the contractor�s facility. The Period of Performance shall be One Base Year, plus Four Option Years. Provision 52.212-1, Instructions to Offerors � Commercial (JUL 2021), applies to this acquisition. �Note, unless exempt per Federal Acquisition Regulation [FAR] Part 4.11, all contractors must be registered and have an active registration in the System for Award Management (SAM) to be considered for an award against this solicitation.� SAM combines data that was formerly contained in the Central Contractors Registration (CCR) and Online Representation and Certifications (ORCA) systems.� For information please review the SAM website at https://www.sam.gov.� A Data Universal Numbering System (DUNS) number is utilized for registration.� To register go to https://www.acquisition.gov. A comparative evaluation will be conducted in accordance with FAR Part 13.106-2(b)(3) to determine the best value.
