Solicitation for GIMC-IHS21-1 Pre-Fabricated Portable Small Buildings For the Gallup Indian Medical Center (GIMC), Gallup, NM 87301 Submit Proposals to stefanie.atene@ihs.gov marked at the subject line ""Confidential GIMC-IHS21-1"" Product Service Code: 5410 NAICS Code: 332311 This solicitation notice is a request for three (3) prefabricated units as per the attached Performance Work Statement (PWS) all inclusive of manufacturing, delivery, installation, labor, equipment, supplies, warranties, and all other related factors. Period of Performance: Final product to be completed and installed in accordance with the PWS. Deliverables: To be delivered in accordance with the attached PWS.�� Price Schedule: Please submit proposals with an itemized price schedule to be reviewed and evaluated.� Attach additional information if necessary. Evaluation Criteria: The Government reserves the right to issue a single award or multiple awards to contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following Evaluation Factors will be considered:�Past Performance, Technical Capabilities, Delivery/Installation Dates, and Price. This acquisition is a Total Small Business set aside. In accordance with 52.204-7(b) (1) prospective contractors must have - and maintain�in accordance with 52.204-13(c) - an active profile in the System for Award Management at beta.SAM.gov in order to submit a quote.
