November 8, 2021 This Request for Quotation (RFQ) is amended as follows: A. Statement of Work 1) Removed Section 5.10�Optional Contractor Annual Support in its entirety,�and all references to it from the Statement of Work. 2) Removed Section�7.2.2�Optional Contractor Annual Support�in its entirety from the Statement of Work. 3) Removed�Section 5.11 Optional Annual Manufacturer (OEM) HW and SW Maintenance and Support in its entirety, and all references to it from the Statement of Work. 4) Removed�ATTACHMENT A-2 Optional Annual Manufacturer (OEM) HW and SW Maintenance and Support from this Request for Quotation, and the entire 2nd paragraph referencing it under Section�7.2.1 Bill of Materials of this Statement of Work. B. ATTACHMENT A� 5) Removed all Option Periods and corresponding�Line Item #s from ATTACHMENT A�SAA Request for Quotation - PRICING TABLE . Please go the Attachment section of this Notice of Amendment for the amended Statement of Work and ATTACHMENT A�SAA Request for Quotation - PRICING TABLE ALL OTHER TERMS AND CONDITIONS OF THIS REQUEST FOR QUOTATION REMAIN UNCHANGED.
