Page 4 of 4 THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 20, is conducting market research to identify potential sources which can provide the following services: The Portland VA Medical Center (VAMC) requires Prospective contractors to provide the following WELCH ALLYN 3.5V HALOGEN OPTH/MACROVIEW Brand Name or Equal per FAR 52.211-6 Brand Name or Equal to Welch Allyn. Item # Part Number Description Qty 1 EM-9477-150M WELCH ALLYN 3.5V HALOGEN OPTH/MACROVIEW Welch Allyn 3.5 V Halogen HPX Diagnostic Set including Standard Ophthalmoscope (#11710), MacroView Otoscope (#23810), Rechargeable 6 0-Minute Power Handle(s), Hard Storage Case; Nickel-Cadmium (#72300) Battery; IEC Plug Type-A. 30 Salient Characteristics The otoscope/opthalmoscope kit must be a compact kit with one handle and interchangeable otoscope/opthalmoscope heads for use by mobile clinicians. The unit must have self-contained charging capability so the unit can be charged wherever a standard power outlet is available, without also carrying a charging cord or charging/docking station. The handle unit must be of durable construction (e.g. nickel-cadmium) to withstand regular transportation and cleaning without breaking. The pieces of the unit (handle, otoscope and opthalmoscope) must have a compact zippered carrying case which contains all of the pieces of the kit. The ostoscopes must have macroview. Requires 3.5V, AC rechargeable adapter built. Requires direct plug into standard outlet for charging. Potential Businesses having the capabilities necessary to provide the above stated equipment services are invited to respond to this Sources Sought Notice via e-mail to derek.crockett@va.gov no later than 11/15/2021. No telephone inquiries will be accepted. RESPONSES SHOULD INCLUDE THE FOLLOWING INFORMATION: company name, address, and business size, point of contact name, phone number, and e-mail address, OEM Certifications for Technicians to provide the services must be included in the response. OEM documentation confirming access to all required OEM parts must be included in the response. Please note whether system is presently offered on a current Government contract. NAICS Code 339112 is applicable to determine business size standard. Any questions or concerns may also be directed to derek.crockett@va.gov via e-mail.
