Synthes Distal Fibula and Radius Instrument Set (Brand name only) INTRODUCTION/BACKGROUND: The Lebanon VA Medical Center has a brand name requirement for one (1) Synthes Distal Fibula Set and one (1) Synthes Radius Instrument set. MAJOR REQUIREMENTS: The Contractor/ Vendor must complete the following tasks to meet the needs of this acquisition: Draft Statement of Need Synthes Distal Fibula and Radius Instrument Set Background: The Lebanon VA Medical Center (LVAMC) has brand name requirement for one (1) Synthes Distal Fibula Set and one (1) Synthes Radius Instrument set to perform Surgical Ankle Trauma procedures. Scope of Work: The Synthes Distal Fibula and Radius Instrument Sets are needed to accommodate a Orthopedic Surgeon who will be repairing ankle trauma in the Operating Room. The Synthes Brand is required due to training and utilization of this type of instrument and implantation devices. No other brand will suffice due to the specific training and knowledge the surgeon has for this type of instruments/implants for this specialty surgery, as well as this item is needed to integrate with other Synthes instruments utilized for this surgery. GC SYSTEM Additional Information: Both the Fibula and Radius Synthes Sets are required for the reasons stated above, and because of their superior design. They are variable angle devices, made of stainless steel, and feature anatomic shape and profile. They have combi holes which provides the benefit of axial compression and variable angle locking capability. They also allow for fixation of small metaphyseal and epiphyseal segments where traditional screw fixation may be limited. The sets are also modular which allows customized selection of implants. Delivery: Delivery shall be within thirty (30) days from the time of award. The Synthes Sets shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through SPS who will receive the items for sterilization prior to use, and the Contracting Officer Representative. This will include documentation of receipt and assignment of a facility based EE number for identification and tracking purposes. Delivery confirmation of non-equipment/ commodities acquisitions is the responsibility of the receiving Facility and/ or Service.
