- THIS IS A SOURCES SOUGHT NOTICE ONLY FOR VAMC WEST HAVEN, CT PURCHASE REQUISITION: 689-22-1-7441-0064: EXCHANGE FOR TWO (2) PHILIPS TEE X8-2T ULTRASOUND PROBE: This announcement is seeking information from the industry, which will be used for preliminary planning purposes for a one-time exchange. No proposals are being requested or accepted with this synopsis. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. THIS IS STRICTLY MARKET RESEARCH. The purpose of this synopsis is to gain knowledge of interest, capabilities, and qualifications of various members of industry, who have the capability and are authorized by the OEM Philips to provide an exchange for TWO (2) Philips TEE X8-27 Ultrasound Probe. This Sources Sought is Brand Name Specific. The work shall include product order, specifically for the designated exchange/delivery, and support as detailed in the SOW included below. The standard warranty will be provided at no additional cost. The vendor must also demonstrate that they are competent and certified by the OEM to provide the exchange and must be able to meet requirements as listed in the SOW below. All technical support and services shall be conducted onsite per the SOW. The contractor will be required to work directly with the Program Manager. The equipment shall always be 100% operational. All equipment must be free of debris, clean, and safe to operate, at all times. All contractors with interest meeting the requirements identified in this notice should submit the attached area of consideration with their response: The eventual vendor must satisfy the following criteria: must be authorized by Philips the OEM to provide the exchange, delivery on or during the week as agreed upon with the customer after the final selection is made, the technical requirement of supplying the brand name equipment as specified, and providing a quote at a fair and reasonable price that affords best value. Prior Government contracts are not required to submit a response under this Sources Sought synopsis. The intent of the Sources Sought is to identify vendors in the open market who can fulfill the requirements as stated in the SOW included below. All work for the resulting contract shall be completed by the selected vendor for this requirement as detailed in the SOW. This notice is published to conduct market research to determine if there is a sufficient number of vendors in the open market who can supply this order in aggregate, while affording competition to the fullest extent possible. This request is urgent and of short duration. The turn time will be short to meet the need. All interested firms must be registered in SAM (System for Award Management) to be eligible for award of Government contracts. Interested parties responding to this Sources Sought request shall submit the following information at a minimum: (1) Company name and address point of contact with phone number, and DUNS number. (2) Documentation relating to capability of performance including installation in a hospital environment, training, and a current point of contact and phone number). (3) Socio-economic status to identify business class as being a large business, SDVOSB, VOSB, small business, Woman Owned, etc. (4) Service Area (5) Any other pertinent company documentation, prior to submission of offers to a solicitation. Any SD/VOSB firms responding to this Sources Sought Notice must also provide proof of registration in the VA VetBiz, prior to submission of offers to a solicitation. This market research is for informational and planning purposes. This is not a solicitation and does not obligate the Government to issue a solicitation. Please be advised the Government will not pay any costs for responses and information submitted. All submissions are acceptable via deborah.reardon@va.gov. RESULTS INFORMATION: Respondents will not be notified of the results and it is at the discretion of the Government to utilize results. Firms responding to this Sources Sought announcement, who fail to meet all requirements as stated above, and who fail to provide ALL of the required information requested, will not be used to help the Government make the acquisition decision, which is the intent of this sources sought announcement. Philips X8-2t Ultrasound Probe A.1 Purpose The purpose of this contract is to ensure the exchange service of two (2) Philips X8-2t ultrasound Probe. Background, MI90-02582A, for cardiology Dept at West Haven, CT VAMC Campus X8-2t is a highly complex medical device used in the diagnostic imaging of patients in the Cardiology Department. Cardiology suite at the West Roxbury campus of the VA Boston Medical Complex. C.1 Contract Objectives C.1.1 Complete exchange service of the X8-2t Ultrasound Probe. C.1.2 Complete this exchange in a timely manner as to not disrupt the flow of patient care beyond necessary limits. C.1.3 Provide documentation to Clinical Engineering upon completion of the exchange service. D.1 Service Location Part will be delivered to Clinical Engineering Department at 950 Campbell Ave West Haven CT 06516-2770.
