Description
THIS IS NOT A REQUEST FOR BID - PLANS AND SPECIFICATIONS ARE NOT AVAILABLE. THIS NOTICE IS BEING ISSUED TO DETERMINE IF THERE IS SUFFICIENT INTEREST FROM QUALIFIED PRIME CONTRACTORS who are Small Businesses, HUBZone small businesses, Woman owned small business concerns, 8(a) small businesses, or Service disabled veteran owned small businesses and who intend to submit a bid for this solicitation to allow for a set-aside in one of these programs. Contractors MUST submit the following by e-mail to CFLAcquisitions@dot.gov for receipt by close of business (4 p.m. local Denver time) on November 29, 2021: (1) A positive statement of your intention to submit a bid for this solicitation as a Prime Contractor; (2) A copy of the letter from SBA stating date of HUBZone small business certification or acceptance to the 8(a) small business program. In the case of a Service Disabled Veteran Owned Small Business Concern, you must provide proof of eligibility (DD form 214 or a letter of adjudication from the Veterans Administration). This information must be provided in order to determine eligibility - DO NOT SEND COPIES OF YOUR SAM PROFILE; (3) Letter from bonding agent stating your firm's capability to bond for a single project of $4 million, and your firm's aggregate bonding capacity; and (4) Provide a list of road construction projects of equal or greater value and scope to the CA ERFO KLAMATH NF 17N11(1)� Upper Doolittle & Mill Creek Bridges project in which you performed (as the prime contractor) removal of structures and obstructions, excavation, embankment, placement of riprap, roadway aggregate, prefabricated steel bridge construction, asphalt concrete pavement, and installation of guardrail. State whether your firm was the prime contractor or subcontractor on the project. DO NOT SEND INFORMATION ON PROJECTS THAT DO NOT INCLUDE THE ABOVE WORK. The size of a small business firm includes all parents, subsidiaries, affiliates, etc. Refer to the Code of Federal Regulations 13 CFR 121.103 for information on how the SBA determines affiliation and 13 CFR 121.108 regarding the penalties for misrepresentation of size status. All firms should be certified in the System for Award Management (SAM) located at https://www.sam.gov/ A Qualified HUBZone SBC receiving a HUBZone contract for general construction must spend at least 50% of the cost of the contract incurred for personnel on its own employees or employees of other qualified HUBZone SBCs. This requirement may be met by expending at least 50% of the cost of the contract incurred for personnel on its employees or it may subcontract at least 35% of the cost of the contract performance incurred for personnel to one or more qualified HUBZone SBCs. A qualified HUBZone SBC prime contractor may not, however, subcontract more than 50% of the cost of the contract incurred for personnel to non-qualified HUBZone SBCs. PROJECT DETAILS: CA ERFO KLAMATH NF 17N11(1) Upper Doolittle & Mill Creek Bridges PROJECT DESCRIPTION:� The Klamath National Forest (KNF) sustained significant damage to transportation facilities, including bridges during the 2020 fire season. The US Forest Service (FS) is requesting that the Central Federal Lands Highway Division (CFLHD) perform engineering and construction services to replace the Upper Doolittle and Mill Creek Bridge structures utilizing Emergency Repair for Federally Owned Roads (ERFO) funds. The Upper Doolittle Creek Bridge is located approximately 5.9 miles Northwest of Happy Camp on Forest Route 17N11 within Klamath National Forest in Siskiyou County, CA.� The Mill Creek Bridge is located approximately 15.3 miles Northwest of Happy Camp on Forest Route 18N15 within Klamath National Forest in Siskiyou County, CA. Upper Doolittle Existing bridge information: Single span structure 41� long with clear width 14.1�.� On tangent with no skew.� Bridge is single lane bridge built in 1966.� Bridge designed to H20 design vehicle load.� Not posted prior to fire. Superstructure is timber glulam girders, deck panels, and railings.� No approach railing.� Object markers at ends. Substructure abutments and wingwalls are timber pile and lagging walls with tiebacks.� Bottom of wall approximately 17� below profile grade. Rock bearing not noted on plans. Mill Creek Existing bridge information: Single span structure 40� long with clear width 20.25�.� Bridge is on a horizontal curve alignment of radius 100� but constructed on tangent chord with no skew.� Bridge is extra wide single lane bridge, most likely to accommodate turning movements of vehicles along sharp curve.� Bridge built in 1981.� Bridge designed to HS20 design vehicle load.� Bridge not posted prior to fire. SCOPE OF THE WORK: The purpose of this project is to fully replace both the Upper Doolittle and Mill Creek Bridges.� Upper Doolittle Proposed Scope/information: A 70-ft x 16-ft proposed structure is anticipated for Doolittle Creek. Prefabricated steel bridge with rolled girders, structural steel decking and asphalt fill.� Structural depth would be approximately 36� deep. All structural steel girders, diaphragms, and railing components on bridge would be weathering steel. Steel thrie-beam bridge railing. Mill Creek Proposed Scope/information: A 60-ft x 28-ft proposed structure is anticipated for Mill Creek. Prefabricated steel bridge with rolled girders, structural steel decking and asphalt fill.� Structural depth would be approximately 36� deep. All structural steel girders, diaphragms, and railing components on bridge would be weathering steel. Steel thrie-beam bridge railing. The roadway approaches will be reconstructed for up to 150 feet on either end of the bridges. The bridges and short sections of the roadway approaches will be asphalt overlaid to reduce sedimentation into the streams. Bridge rail and guardrail with terminals will be included. Items worth noting: Both existing structures have been completely removed and the timber stacked on site. The FHWA contract will include disposal of the material. The work will be listed within a single Schedule (Schedule A) and no options are proposed. Adjacent segments of the Forest Routes will remain closed during construction and can be used for contractor staging and storage.� Access is to both sides of the Doolittle Creek Bridge via by an approximate 8-mile detour. Access is to both sides of the Mill Creek Bridge via by an approximate 10-mile detour.
