Description

The Indian Health Service, Gallup Service Unit, located in Gallup, New Mexico, 87301 has an upcoming requirement for one (1) Full-Time Non-Personal Healthcare Services for Physicians in Family Medicine/Palliative Care Department. The requirement will include a base period plus three�3-month option periods. The North American Industry Classification System or NAICS code is 62111 with a small business size standard of $12.0 M.� The Request for Quotations No. 75H71022Q00005 using the principles of FAR 12 Acquisition of Commercial Items in conjunction with FAR 13 Simplified Acquisition Procedures will be issued on or by 11/15/2021.� Evaluation factors may include: 1. Ability to manage complex patients 2. Contractor�s experience of three (3) years beyond residency training. 3. Contractor�s Licenses/Certifications (BLS, medical license, Diplomas etc.), Board Certified/Board Eligible & Valid Driver's License 4. Competency in Family Medicine and Palliative Care 5. Interview with Contractor against Performance Work Statement, and Contractor Profile 6. Provider must be available to start as soon as possible 7.�Submittal of Option CLIN per Fee Schedule 8. Price Quotes will be due on or by 11/23/2021.� This acquisition is unrestricted.
