- Please note this requirement is set-aside for SMALL BUSINESSES only.� The Contractor shall provide all personnel, tools, equipment, labor, and travel necessary to ensure that CUI document shredding services are performed at the Warner Robins Air Logistics Complex (WR-ALC) in accordance with applicable laws, regulations, and authorities. NOTE: There is no contract ceiling price. The contractor's maximum obligation amount is a system generated error. BPA AGREEMENT SCHEDULE Blanket Purchase Agreement Terms and Conditions IAW FAR 13.303-2 and 13.303-3 (1) Description of agreement: The contractor shall furnish a quote for services, if and when requested by the contracting officer (CO), authorized CO representative, or one of the individuals authorized to place call/orders against this BPA during the period specified in the clause below entitled ""Effective Period"". Call/orders made hereunder shall be written. Such call/orders shall include a request for quote sent to the BPA participant, description of the services being ordered, delivery schedule, FOB point, place of inspection and acceptance, and designation of appropriations chargeable together with such other specifics covered elsewhere herein. (2) Extent of obligation: The Government is obligated only to the extent of authorized call/orders actually placed against this Blanket Purchase Agreement. (3) Purchase limitation. No Individual call/order under this agreement shall exceed $250,000.00. (4) Individuals authorized to purchase under the BPA: A list of names of individuals authorized to place call/orders under this BPA is attached to this agreement per Section J. (5) Delivery tickets. A requirement that all shipments under the agreement, except those for newspapers, magazines, or other periodicals, shall be accompanied by delivery tickets or sales slips that shall contain the following minimum information: (i) Name of supplier. (ii) BPA number. (iii) Date of purchase. (iv) Purchase number. (v) Itemized list of supplies or services furnished. (vi) Quantity, unit price, and extension of each item, less applicable discounts (unit prices and extensions need not be shown when incompatible with the use of automated systems, provided that the invoice is itemized to show this information). (vii) Date of delivery or shipment. (6) Invoices: A summary invoice shall be submitted by the vendor to the Contracting Officer (AFSC/PZIM) for all services provided against the BPA on a monthly basis. At a minimum, the invoice shall include: company name and address, invoice date and number, BPA number, itemized description of services provided (which includes organization receiving services and cost of services) (7) Effective Period of Performance: This BPA is expected to be effective from 4 December 2021 - 3 December 2026. (8) This agreement may be discontinued and/or canceled upon 30 days written notice by either party. Instructions:� The Government proposes to establish a Blanket Purchase Agreement (BPA) for Controlled Unclassified Information (CUI) Document Shredding. 1. This Solicitation document FA8571-22-Q-0015 will contain the Terms and Conditions desired. The Pricing Spreadsheet attachment is a Representative Sample of the Controlled Unclassified Information (CUI) Document Shredding Services being requested. Evaluation factors can be found in Section M of this Solicitation document. 2. Either party may choose to terminate this agreement provided a 30-day notification is given. The Government will establish one BPA. The Government intends to have this BPA in place and effective beginning December 4, 2021. If your company is selected to establish an agreement, the government will notify you in writing for additional information. This information will include how calls are placed, as well as a list of individuals who are authorized to place calls for this proposed agreement. 3. Please complete all representations and certifications on this solicitation document and return with your responses. Your Completed Solicitation document and the filled out Pricing Spreadsheet attachment must be emailed to: thomas.royal.1@us.af.mil no later than 19 November 2021, 5:00 p.m. EST.
