- 612-163 Construct Social Work Consolidated Clinic Space Project #: 612-163 GENERAL INTENTION A. Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for the construction of a new 14,568 square feet, two-story structure for the consolidation of Social Work, Home Based Primary Care, and Home Telehealth support on the Northern California Health Care System, Mather Campus at the department of Veterans Affairs, VA Mather campus, 10535 Hospital Way, Mather, California 95655 as required drawings and specifications. Period of performance for construction shall be 500 days. B. Visits to the site by Bidders may be made only by appointment with the Contracting Officer. C. Offices of Romanyk Consulting Corp, as Architect-Engineers, will render certain technical services during construction. Such services shall be considered as advisory to the Government and shall not be construed as expressing or implying a contractual act of the Government without affirmations by Contracting Officer or his duly authorized representative. D. Before placement and installation of work subject to tests by testing laboratory retained by the contractor, the contractor shall notify the COR in sufficient time to allow COR witnessing of testing laboratory personnel proper taking and testing of specimens and field inspection. Such prior notice shall be not less than three work days unless otherwise designated by the COR. E. All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police. 1.2 STATEMENT OF BID ITEM(S) BASE BID (A. 1., 2.) ITEM I (BASE BID), GENERAL CONSTRUCTION TO COMPLETE PROJECT NO. 612-163 Design Social Work and Expand Clinical Space"": (SEE ALSO 6 BID ADD ALTERNATES BELOW) Work includes general construction, alterations, temporary roads, walks, grading, drainage, site work, landscaping, and necessary clearing of existing site conditions. Structural Work and Framing: Work includes foundations, structural framing, and all interior and exterior partitions for the project as called for in the drawings and specifications. Electrical Work: Work includes all labor, material, equipment and supervision to perform the required electrical construction work on this project including new power to the site, transformers, lighting, low voltage alarm, card key access, and all work indicated on the drawings and specifications to provide a fully functioning electrical system for the project. Mechanical and Plumbing Work: Work includes all labor, material, equipment and supervision to perform the required Mechanical and Plumbing construction work on this project including all HVAC units, ductwork, plumbing, connections to utilities, fire sprinkler piping and all work indicated, on the drawings to provide a fully functioning mechanical system for the project. Base bid also includes the demolition of Building 800 (B800) as described on sheet AD101 in the drawing set. 6. BASE BID $__________________________________________________________(BASE BID AMOUNT ONLY) B. ALTERNATE NO.1: Provide all work associated with the drawings and as indicated in the Base Bid above AND to include all work necessary to install screen walls around roof mounted mechanical equipment (Basis of design is CITYSCAPE Envisor Screen roof mounted System with relevant engineering by the manufacturer or VA Approved Equal). This will require structural attachment of columns to support the screening and roof penetrations. W12x50 beams will be required to support mechanical enclosure post and kickers. Panels will be 70 Louver Panel with band trim 1 -0 off of the roof. See Roof Plan for area to be enclosed (APPROXIMATELY 96 -4 x 48 ). (see ADD ALTERNATE INDEX on the cover sheet for the drawings for locations of details (see AE 105)) Also in the Alternate include the millwork work station (see ADD ALTERNATE INDEX on the cover sheet of the drawings for locations of details) as indicated on the drawings for the South end of the second floor. $ _________________________________________(ADD ONLY) PLUS BASE BID AMOUNT ABOVE. C. ALTERNATE NO. 2: Provide all work associated with the drawings and as indicated in the Base Bid above AND to include all work and materials necessary to install decorative metal panels in lieu of the EIFS/Stucco. The same joint pattern will apply to the metal panels as are shown on the drawings for the EIFS/Stucco. (see ADD ALTERNATE INDEX on the cover sheet of the drawings for locations of details (see AE 101)) _ $_____________________________________________(ADD ONLY) PLUS BASE BID AMOUNT ABOVE. D. ALTERNATE NO. 3: Provide all work associated with the drawings and as indicated in the Base Bid above AND to include all work and materials necessary to install an aluminum cantilevered canopy at the main building entrance to the H/C parking (approximately 60 long 8 -0 wide and 10 -0 tall at the low point), and another along the exterior storage and locker area (approximately 37 long x 6 -0 wide). This will need to include new structural supports, concrete footings, material, labor and construction/ installation. Also, include lighting on a separate photocell for the canopy from the entrance to the parking. Provide a system by AVAdek this is a pre-engineered and designed system or VA Approved Equal (see detail on AS101) (see ADD ALTERNATE INDEX on the cover sheet of the drawings for locations of details (see AS 101) $_____________________________________________________________(ADD ONLY) PLUS BASE BID AMOUNT ABOVE. E. ALTERNATE NO. 4: Provide all work associated with the drawings and as indicated in the Base Bid above AND to include all work and materials necessary to install larger caliper landscaping trees (4 caliper +), fully mature bushes, fully mature shrubs, and larger boulders (boulders should be aged, and lichen moss covered). Increase boulders from 48 to 60 and 36 to 48 . Increase number of large rocks to 25 and smaller rocks to 60. Change gravel areas to artificial turf. (see ADD ALTERNATE INDEX on the cover sheet of the drawings for locations of details (see AS 101)) $ __________________ ___________________________(ADD ONLY) PLUS BASE BID AMOUNT ABOVE. F. ALTERNATE NO. 5: Provide all work associated with the drawings and as indicated in the Base Bid above AND to include all work and materials necessary to install decorative Natural Stone veneer wainscot, with L shaped corner stones, 3 -0 high around the entire first floor with a cast stone water table at the top. (see ADD ALTERNATE INDEX on the cover sheet of the drawings for locations of details (see AE 101)) $ _______________________________________(ADD ONLY) PLUS BASE BID AMOUNT ABOVE. G. ALTERNATE NO. 6: Provide all work associated with the drawings and as indicated in the Base Bid above AND to include all work and materials necessary to install the parking shown on the drawings as future parking (approximately 18,200 SF) from the entrance at Femoyer to the entrance at the future extension of McCuen Blvd. This will include concrete curb and gutters (approx. 500 LF), asphalt paving over gravel sub-base, parking striping, drainage features as required, dumpster pad, and parking lot lighting for a fully functional parking lot area. General Contractor to provide water release and flow from parking to adjacent property and/or street. General Contractor to provide all permits for storm water release from property to adjacent property and/or street(s). Water release beyond perimeter of parking lot curb and gutter is Not in the Contract (NIC). ADD an ALLOWANCE of $45,000 for Civil Engineering design of the grading and drainage of the parking. (see ADD ALTERNATE INDEX on the cover sheet of the drawings for locations of details (see AS 101)) $_____________________________________________________________(ADD ONLY) PLUS BASE BID AMOUNT ABOVE. SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR A. Drawings and contract documents may be obtained from the website where the solicitation is posted. Additional copies will be at Contractor's expense. B. Records Management Requirements: The following standard items relate to records generated in executing the contract and should be included in a typical Electronic Information Systems (EIS) procurement contract: Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
