Description
I. DESCRIPTION OF SERVICES: GENERAL: The contractor shall provide all needed labor, materials, parts (to include system components), Supervision, Project Management, Equipment, Quality and Safety Assurance, and all other necessary resources required to replace three exterior doors (main entrance, ER entrance, & MRI entrance) on building 1 at VA Northern Indiana Health Care System on the Fort Wayne Campus located at 2121 Lake Avenue, Fort Wayne, IN 46805. The campus buildings are pre-1950 construction. Asbestos and lead containing paint are present in building materials. The VA shall provide the most recent surveys of hazardous material. The contractor shall provide all mitigation/remediation as applicable to complete the project. As a working hospital the adjacent spaces and functions will necessarily need to be maintained and a high degree of coordination, scheduling, and phasing will be required to allow the continuation of the existing hospital services during the entire construction project. SCOPE: To include: Contractors work and responsibility shall include all contractor planning, programming, administration, and management necessary to provide all construction and related services as specified. Contractor shall provide all related contractor administration services to perform the work such as quality control, contractor financial control, preparing and submitting required reports, and submitting necessary information as specified under this contract. The work shall be conducted by the contractor in strict accordance with the contract, to include all applicable federal, state, and municipal laws, codes, regulations, and directives and specifications. The contractor shall coordinate all shutdowns with the COR at least 3 weeks in advance of the shutdown. This shall include interior and exterior such as work involving cranes, taking parking spaces, utilities, and public spaces. Representatives of the contractor shall be required to attend a Pre-Construction Conference prior to scheduling work, mobilizing, or performing any work for any issued request. The Pre-Construction Conference shall be conducted by the CO or the designated COR and will consist of a formal review of project requirements and establish the Notice to Proceed (NTP). Contractor shall provide an appropriate means of disposing of all waste from remodel. Contractor shall provide an inventory/record/log of waste items by weight to the VA. Contactor shall remove any lead-based paint from all windowsills and/or door frames and dispose of properly. Work Shall be performed after 3pm Monday Friday or on weekends to minimize patient interruption. The contractor shall coordinate project expectations with the subcontractors to ensure everything in the SOW and on the specifications and drawings is included in subcontractor bids and work is properly completed prior to the final inspection. Contractor shall provide a site-specific safety plan. Contractor shall provide a construction schedule before the start of work. Contractor shall provide Submittals to COR prior to install. Main entrance o Consists of two dual external and two dual internal sliding glass doors o Replace the two dual internal sliding doors with two single sliding door packages to include all the glass fixed side-lite dark bronze finish doors located at the edges of the opening full break-out configuration narrow stile 7 bottom rail standard Mortise lock full jamb to jamb threshold two overhead combination activation and safety sensors concealed adjustable bottom sweep ¼ glass pocket side-lite will be in between the two single door packages 110 VAC power sliders 208 VAC 3 phase heated air curtain and intelli-switch and thermostat Replace the two dual external sliding doors with one Bi part sliding door package to include all the glass fixed side-lite on each side dark bronze finish centered in the opening narrow stile with 7 bottom rails standard Mortise locks full jamb to jamb threshold two overhead combination activation and safety sensors concealed adjustable bottom sweeps 1 glass pocket 110 VAC power sliders ER entrance Consists of one dual external and one dual internal sliding glass door Replace both sets of doors with new external and internal sliding glass doors to include all the glass Bi part sliding door Full beak out configuration Insulated glass for exterior ¼ tempered glass for interior Electronic locking for exterior Concealed adjustable bottom sweeps Narrow stile 7 bottom rail Two combination sensors MRI entrance Consists of two bi-folding glass doors Replace doors with new sliding glass doors to include all the glass Three panel telescoping sliding glass door Dark bronze finish Narrow stile 7 bottom rails Two ANSI 156.10 sensors Intermediate horizontal muntins Five position key switch for on/off/hold/open reduced/open exit only Reuse 110 VAC power Clear insulated 5/8 glass & caulk
