- Page 1 of Page 1 of Page 1 of Solicitation Number / RFQ 36C24422Q0125 Annual Wall to Wall Pharmacy Services, Wilmington VA Medical Center, Wilmington, DE In accordance with FAR, this is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The combined synopsis solicitation number is 36C24422Q0125. It is issued as a Request for Quote (RFQ). The government anticipates awarding one firm-fixed price contract resulting from this solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-07 (09/10/2021) and can be found at http://www.acquisition.gov/far/index.html The North American Industrial Classification System (NAICS) code for this procurement is 561990 with a small business size standard of 11 Million annually (yearly). This solicitation is set-aside 100% to Small Businesses. This solicitation requires vendors be registered with the System for Award Management (SAM) at the time of quote response date pursuant..."" to applicable regulations and guidelines. Registration information is at www.sam.gov. Confirmation of SAM registration will be validated prior to awarding a contract. The Department of Veterans Affairs Medical Center (VAMC) in Wilmington, DE requires a contract for yearly inventory to be performed for both inpatient and outpatient pharmacies for the Wilmington VA Medical Center, which is locates at 1601 Kirkwood Highway. Wilmington, DE. This includes the counting of all drugs and supplies in the main pharmacies as well as the Script Pro and Omnicell machines. There is no pharmacy in-house count and pharmacy staff will only be supervising, not assisting. The Contractor shall provide all the resources required to perform the financial courier services as outlined in the Statement of Work, Attachment A. Wage Determination 2015-4215, Revision 14 dated 07/21/2021 applies and can be found in Attachment C. Addendum to FAR 52.212-1 Supplemental Instructions to Offerors applies to this acquisition: For the purposes of evaluating competing proposals, Offerors shall submit proposals that include the following documentation: Addendum to FAR 52.212-1: Offerors shall submit Pricing, Technical Capability and Past Performance information as follows: Submission Information/Instructions: Offerors shall submit their proposal electronically in four volumes. Volume 1 Introductory, Volume 2 Price, Volume 3 Technical Capability, and Volume 4 Past Performance. Failure to follow these instructions may result in Offerors being determined as non-responsive and be removed from this competition. Volume 1: Introductory (cover letter) shall be submitted in Adobe or MS Word Volume 2: Price - the Price/Cost Schedule shall be submitted in Adobe or MS Word Volume 3: Technical Capability - shall be submitted in Adobe or MS Word Volume 4: Past Performance - shall be submitted in Adobe or MS Word All Adobe and MS Word documents shall be submitted with Times New Roman size 12 fonts. Margins shall be no smaller than 1 . A. Volume 1 Introductory Folder (No Page Limit): The Introductory Folder shall include the information listed below and any other items for which a location has not otherwise been specified. Offerors shall acknowledge Limitations on Subcontracting. a. Cover Letter. Offerors shall submit a cover letter identifying their Company name, contact information, CAGE Code, DUNS number of the prime and any teaming partner/subcontractor. b. Page 1 of SF 1449 Complete Blocks 17a. (including Contractor Point of Contact); 30a; 30b. and 30c. c. Page 2 of SF1449 Complete Contractor POC Information and acknowledge amendments, if applicable. d. FAR 52.212-3 Offeror Representations and Certifications-Commercial Items. Offerors shall include a completed copy of the provision with their offer if the Offeror has not completed electronically in the System for Award Management (SAM) accessed through https://www.sam.gov/SAM/. 852.219-74. f. All proposals shall be valid 60 days from the quote response date. B. Volume 2 Price a. Offerors shall complete the Price/Cost Schedule in Section B.2. See Attachment B. C. Volume 3 - Technical Capability (No Page Limit): Quoters shall include the following information: A complete technical proposal that demonstrates their ability to perform the work described in the Statement of Work (SOW) Quoters are cautioned to be responsive to all of the requirements in the SOW and to provide sufficient information to allow for evaluation of the quotes. b. Copy of any licenses, permits, and insurance certifications required by law. D. Volume 4 - Past Performance (No Page Limit) Offeror must demonstrate experience providing courier service by identifying three (3) references for whom contracts of similar scope, size and complexity are either ongoing or were completed within the past three years. Include the following information: Company name and address Name, telephone number and e-mail address of Company POC Dates of contract performance, aggregate award amount, and Contract number Past performance information shall be recent (performance within the last three years) and relevant to this scope of work. The Government reserves the right to seek past performance information from any sources not included in your past performance references and available to the Government to include, but not limited to, CPARS or other databases; interviews with Program Managers, customers, and Contracting Officers. Evaluation Criteria for Award FAR 52.212-2 Evaluation Commercial Items (OCT 2014) applies to this acquisition: (a) The Government will award a contract resulting from this solicitation to the responsible Quoter that represents the best benefit to the Government at a price that can be determined reasonable. The Government will evaluate technical capability, past performance and price. The Government will perform an evaluation using lowest price technical acceptable (LPTA). The Government will select the contractor that best benefits the Government by fulfilling the requirement. Responsibility determinations will be made accordance with FAR 9.1, Responsible Prospective Contractors. The Contracting Officer will award one, firm fixed price contract resulting from this RFQ to the responsible offeror whose quote conforming to the RFQ is most advantageous to the Government. (b) A written notice of award or acceptance of a quote, mailed or otherwise furnished to the successful Quoter within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
