Page 6 of 6 Sources Sought/Request for Information (RFI) is issued solely for informational and planning purposes only. It does not constitute the issuance of a solicitation or a promise to issue a solicitation in the future. The Department of Veterans Affairs (VA), James A. Haley Veterans Administration Medical Center (JAHVAMC), 13000 Bruce B. Downs Blvd, Tampa, Florida 33612, is currently seeking information from vendors with the capability to provide preventative maintenance, repair, inspection and installation services on ceiling mounted patient lift, lift gates, rails, and turnstiles. A certified technician must be able to perform maintenance and repair on all equipment regardless of the manufacturer. The applicable NAICS for this service is: 811219, Other Electronic and Precision Equipment Repair and Maintenance, Size Standard: $22,000,000.00. Vendors must be registered in the System for Award Management SAM.gov | Home prior to award. Equipment: Estimated quantities of installed and inventory stock is approximately 1,470 patient lifts, lift gates, rails, and turnstiles systems and the manufacturers are: ARJO, LIKO, Guldman, Getinge and Tollos. Below are locations requiring services. located at the below listed facilities. James A Haley Veterans Hospital and associated Clinics, 13000 Bruce B. Downs Blvd. Tampa, FL 33612. Lakeland Community Based Outpatient Clinic, 4237 South Pipkin Rd Lakeland, FL 33811. New Port Richey Outpatient Clinic, 9912 Little Road, New Port Richey, FL 34654. James A. Haley Veterans' Hospital Primary Care Annex, 13515 Lake Terrace Lane Tampa, FL 33637. Zephyrhills Community Based Outpatient Clinic, 6937 Medical View Lane, Zephyrhills, FL 33542. Orlando VA Medical Center and Clinics, Orlando, Florida VA Caribbean Healthcare System and Clinics, San Juan, Puerto Rico The Contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved service personnel for each model the Contractor services at the JAHVAMC to the Contracting Officer s Representative (COR). Contractor shall revise this documentation whenever service personnel are assigned or re-assigned to the JAHVAMC. The COR reserves the right to request authentication of factory training certificates or credentials from the Contractor at any time for any personnel who are servicing JAHVAMC equipment. The contractor will not require access to VA sensitive information, therefore the Certification and Accreditation (C&A) requirement in accordance with (IAW) VA Handbook 6500.6 Contract Security does not apply. II. SCOPE Contractor must furnish all necessary labor, supervision, equipment, tools, materials, repair service, software updates, parts, weight testing, etc., preventative maintenance, repair, inspection, and installation service of ceiling mounted patient lift, lift gates, rails, and turnstiles systems as specified with manufacturers recommendations, terms, conditions and schedule of this Performance Work Statement (PWS). Contractor shall be required to sign in with the COR after contract award, before beginning a service call and when the COR is not available and during Weekends, Holidays, Evenings and Nights Hours (WHEN), the Contractor shall sign in/out at the Central Energy Plant Building# 39. Upon completion of Preventive Maintenance (PM) inspection the Contractor shall leave a copy of the service report with the using service, and submit to the same location as designated above a service report annotating what service, calibration, etc., were performed and whether preventive maintenance was done. Each service report shall include the name of the contractor, name of Field Service Engineer (FSE), date, time (starting and ending), equipment downtime, hours of labor including costs (when emergency service is authorized outside of normal working hours), the parts utilized including costs, the nature of the problem, the nomenclature of the item replaced (serial number & VA inventory number), and the room number where equipment is located. The service designee and the FSE attesting that the equipment is in satisfactory working order and is functioning at an acceptable level of performance shall sign this report. The contractor shall sign out at the designated location before leaving. NOTE: Failure to furnish a service report or obtain a signed copy of the service report could result in delay of payment or non-payment. The COR or alternate COR has the authority to approve/request a service call from the contractor. Preventive maintenance (PM): Certification and semi-annual inspections/Preventative Maintenance (PM) are scheduled through the COR. The contractor shall perform PM service to ensure that equipment listed in schedule functions in conformance with the latest published editions of NFPA, OSHA, CDRH. The contractor shall provide and utilize procedures and checklists with worksheet originals indicating work performed and actual values obtained (as applicable) provided to the COR at the completion of the PM. PM procedures will be submitted to the COR for approval prior to the initiation of this service contract. PM Services shall include but not be limited to the following: Cleaning of equipment Reviewing operating system software diagnostics to ensure that system is operating to the manufacturer s specifications. Calibrating and lubricating the equipment. Performing remedial maintenance of non-emergent nature. Test and replacing faulty and worn parts and/or parts which are likely to become faulty fail or become worn. Adjusting and calibrating as necessary. Inspecting electrical wiring and cables for wear and fraying. Inspecting all mechanical components including, but not limited to patient restraints and support devices, chains, belts, bearing and tracks, interlocks, clutches, motors, mechanical integrity, safety, and performance being priority Performing Electrical Safety Inspections, which should include line cord ground wire resistance and leakage as specified in the Conformance Standards. PERIOD OF PERFORMANCE: The contract will be performed for five years (one base year and four-one-year options). REPONSES Responses to this Sources Sought/Request for Information Notice shall be able to demonstrate the company s capability to responsibly deliver the services listed in the Performance Work Statement (PWS). The purpose of this Special Notice announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified vendors interested and capable of providing the services described in the PWS. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. No funds have been authorized, appropriated, or received for this effort.
