Description
The Uintah & Ouray Service Unit (U&OSU) provides health care services to the Native American and Alaskan Natives in Utah. The Fort Duchesne Health Center Laboratory provides crucial information to the Providers in order to provide accurate care to the Native American and Alaskan Natives of our communities. We perform approximately 75,000 in house testing per year including SARS-CoV-2. In order to continue to provide testing to support the clinic we must have a CMS license to perform laboratory testing in compliance with the Clinical Laboratory Improvement Amendments (CLIA) regulations found under CFR, Title 42, Part 493 � Laboratory Requirements and maintains a Proficiency Testing Program with annual enrollment with a CMS approved proficiency testing provider as required to maintain accreditation and/or CMS licensure as defined in the CFR and in compliance and/or The Joint Commission (JC) standards QSA.01.01.01 though QSA.01.03.01 for Laboratory and Point-of-Care Testing. Each laboratory and/or POCT site must notify CMS and JC immediately if they are unable to enroll in a proficiency testing program by the beginning of the calendar year also if the proficiency testing provider has changed from the previous year, including why that change occurred. PERIOD OF PERFORMANCE: Performance of this contract shall begin on 01/01/2022 and shall not extend beyond the completion date of 12/31/2026, unless the period is changed by written modification of the contract. STATEMENT OF WORK: The Contractor shall furnish all necessary supplies to provide proficiency testing services to the Uintah & Ouray Service Unit (U&OSU) as mandated by the Clinical Laboratory Improvement Amendments (CLIA) regulations found under CFR, Title 42, Part 493 � Laboratory Requirements and The Joint Commission (JC) standards QSA.01.01.01 though QSA.01.03.01 for Laboratory and Point-of-Care Testing. The Contractor shall be a CMS approved proficiency testing provider as required to maintain accreditation and/or CMS licensure as defined in the CFR and in compliance with the JC standards. Specimen Transport Requirements � The Contractor shall ship proficiency testing kits on a scheduled basis over the span of a calendar year so that the laboratory receives the CMS required number of regulated (and unregulated) analytes, to ensure compliance with above mentioned regulations and standards. The proficiency testing provider shall ship kits packaged so they arrive at appropriate temperatures to maintain stability of testing kits. The Contractor will notify the laboratory once a kit is shipped. Result Reporting and Graded Results � The Contractor shall allow for online submission of results and online graded result reporting.
