Quillen Veterans Affairs Medical Center STATEMENT OF WORK IDIQ COVID Screeners SCOPE The contractor shall provide all necessary labor, supervision, uniforms, safety apparel, FRS Radios to provide on demand timely patient COVID screener services in accordance with the terms and conditions of this statement of work at James H. Quillen Veterans Affairs Medical Center (JHQVAMC), Dogwood Avenue, Buildings 69, 200, 205, 204, 162, 160 and several outdoor screening stations and gates at Mountain Home, Tennessee, 37684. Quality performance of this contract is pivotal in JHQVAMC s ability to improve and sustain no or low staff and patient infection control rates, satisfaction, and appointment utilization rates. CONTRACTOR RESPONSIBILITIES 1. Courtesy: COVID screener employees shall be courteous and cordial to all patients, their family members, their escort, other contracting staff/co-workers and the JHQVAMC staff. The COVID screener contractor shall treat all patients and staff with respect, safety, and with competence. a. COVID screener contractor will assist patients accessing JHQVAMC by screening them and the occupants of their vehicle for COVID according to the Centers for Disease Control (CDC) COVID screening JHQVAMC specific established instructions. Coronavirus Disease 2019 (COVID-19) | CDC b. COVID Screener staff will be asked to screen patients by following a specified protocol of questions supplied by JHQVAMC. Screener staff will be asked to assist in traffic control to limit through traffic and redirect non-essential people away from JHQVAMC. Services are required on weekend days and on official Government holidays. During non-critical hours, screeners could be tasked to assist Veterans with mobility issues to pick up and drop off from the parking lot to the main building. 2. Safety: Screener Contractor employees shall adhere to all state and Federal traffic safety laws, JHQVAMC patient safety and facility safety rules, policies, regulations. Screener Contractor employees shall maintain a valid driver's license with no historical record of traffic violations for DUI, driving under suspended or revoked license or reckless driving. Discovery of a contractor employee with a violation on his/her driver s license will require the Government to disallow the contractor employee on site. Breaks and lunch must be taken away from the screener area of operations. Screeners must wear an approved mask and eye protection while on duty. 3. Security: The contractor shall ensure all employees process through the JHQVAMC Personal Identity Verification (PIV) Office and undergo a Special Agency Check (SAC) (Federal Information Processing Standards Publication 201-1, change 1 notice, dated March 1, 2006 and all subsequent updates or modifications), shall undergo a background screening in accordance with VHA Directive 0710 and shall be qualified to receive and wear a PIV badge to be eligible to work under this contract. Should a prospective applicant be found ineligible to receive a PIV badge, that person will not be allowed to work at JHQVAMC under this contract. Screener contractor employees shall report sighting of any type of weapon in a vehicle or on a person directly to the JHQVAMC Police Service immediately. All contract employees will wear their PIV Badge visible to all people while on the Mountain Home station, no exceptions. Contractor employees will always wear a designated professional uniform while on station. 4. Competency/Training/License/Qualifications: The contractor shall provide fully trained, experienced screener who possess a valid driver s license and are legal U.S. citizens. Contractor shall ensure all employees maintain their license with no historical record of traffic violations for DUI, driving under suspended or revoked license or reckless driving. Contractor employees shall be at least 18 years of age and be able to perform all duties, functions, and activities as required. Contractor employees shall read, write, and speak English clearly. English is the only acceptable language for use on duty. Contractor is responsible to maintain employee s competencies clearly documenting each employee can conduct the screening. All COVID Screeners are required to complete Health Insurance Portability and Accountability Act (HIPAA) and Privacy training annually. All COVID Screeners must complete this training prior to performing duty. The Government will provide a training packet and each contractor employee will read, sign and date that they understand and will abide by the instruction provided as required. This instruction will include Safety, and Occupational Safety and Health Administration, Security, Emergency Procedures, accidents/injuries, and communication with JHQVAMC staff. The contractor shall include compliance with this training in their employee competency documentation showing the employee accomplished this training annually. The primary role is to protect, sustain, and nurture our Veteran patient population. Contractor employees are required to provide health screening to rule in or out COVID-19 and other upper respiratory conditions and issue masks to those without a mask. Employees (Screeners) will tag each screened patient who screens negative with a sticker identifying them as screened for that day. Screeners will be knowledgeable about JHQVAMC to direct Veteran patients to the appropriate buildings for their appointments and ensure only one attendant accompanies the Veteran to the appointment. Screeners will direct those who respond positively to initial COVID-19 or upper respiratory screening questions to detailed medical screening and ensure they go to the correct location for detailed medical screening/testing. The contractor s onsite supervisor will track and manage ordering of supplies and delivering supplies to screener stations. The supervisor also ensures screeners wear their masks and eye protection, wash their hands frequently and prudently utilizes supplies. 5. Tobacco and Food: Use of any tobacco products, vaping or e-cigarette at JHQVAMC is prohibited, including use in privately owned vehicles. Food will be consumed in break areas only and not at screener, gate or door stations. COVID Screener must socially distance while eating. 6. Uniform Standards: Screener contractor shall provide employees with distinctive bright colored professional uniforms containing the contractor s logo for quick easy identification. Uniforms will include a high visibility class 3A ANSI 207 bright colored reflector safety vest to be worn for duty and the uniform shall consist of pants, shirt, cap or visor, raincoat, rain pants, overcoat, gloves, and watch cap as appropriate for weather. The high visibility vest will always be worn on the outside, incorporated in the clothing item or worn on top of all other clothing. Personal clothing is not acceptable on duty. The Contracting Officer (CO), Project Manager (PM), and Contracting Officers Representative (COR) are the approval authority for uniforms. Jewelry and eyewear shall be conservative when worn and present a professional appearance. PIV Badges will always be worn visible to the public as part of the uniform while on duty. 7. Traffic Control: COVID Screeners shall serve to ensure management of the flow of traffic around the Mountain Home Station experiencing no traffic backup or jams in the screener area of operations. The screeners will notify the JHQVAMC Police Service immediately of a traffic flow stoppage or backup. Screeners will socially distance around Veteran vehicles and avoid touching the vehicles. 8. Reporting Requirements: The contractor shall provide written reports as follows: a. Report any shift or station that cannot be filled by 7 a.m. the day the shift is scheduled. b. A copy of timesheets will be submitted with the monthly invoice. The timesheet shall show hours worked by day of the week and any change based upon more than 40 hours worked including overtime worked. 9. Demand Management: The volume of Veterans traveling through screener stations should grow. The contractor should anticipate a growing and varying daily demand; therefore, employ an on-call should already be trained and ready to perform on short notice to respond to the varying ups and downs of a growing demand and to fill in during annual and sick leave periods for other employees. This on-call pool will be activated upon the contractor receiving notice of termination by another employee. The important point is to maintain staffing levels to meet the demand and not experience low staff with high demand for screening. The historical data shows a minimum of 25 employees to a maximum of 32 employees are needed to accomplish this task. The objective is no Veteran gets into the facility without being screened. A person accessing the medical center facility without being screened may cause significant risk to patients and staff. FACILITIES 1. The Government will provide a screener booth, telephone in the booth and outside the booth (only local calls are authorized), traffic flow signage, 18 wall lockers (2 rows of 9 double stacked), outdoor screening stations, and indoor screening stations. JHQVAMC will provide snow removal and deicer to promote safe operations area during winter months. The government is responsible to maintain the equipment they provide. 2. The contractor will provide office supplies and material, large umbrellas, and miscellaneous items necessary to operate an efficient screener service. HOURS OF OPERATION Contractor shall be operational on or before the hours of 5 a.m. to 9 p.m., Monday through Friday, including Federal holidays. Contractor shall be operational on or before the hours of 6 a.m. to 9 p.m., Saturday and Sunday. Listed below are the ten national holidays: New Year s Day January 1 Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Juneteenth June 18 Independence Day July 4 Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11 Thanksgiving Day Fourth Thursday in November Christmas Day December 25 The Contractor shall be notified in advance of any special events which might cause screener problems as a result of additional patients, patrons, or visitors to JHQVAMC. The Contractor shall provide increased staffing or staff the demand during these periods as scheduled. The government will provide a monthly schedule generally before the end of the fourth week of each month outlining the need for the next month. Place of Performance: Department of Veterans Affairs James H. Quillen VAMC Corner of Lamont & Veterans Way Mountain Home, Tennessee 37684 GENERAL INFORMATION The contractor shall ensure all contract employees provide the following documentation: 1. PPD Skin Test: Annual test is required. Individuals must provide documentation of receiving the PPD skin test. 2. Annual Flu Shot Annual flu shot is required. Individuals must provide documentation of receiving the annual flu vaccine. It shall be the responsibility of the contractor to provide documentation of the above prior to assignment or hiring and during employment. 3. Drug/Alcohol Policy: Contractor shall ensure that all contractor staff performing under this contract complies with the JHQVAMC alcohol and drug free workplace policy. The contractor shall be expected to conduct random alcohol and drug tests on all staff. Random alcohol and drug tests shall be conducted at least once a year on each employee performing under the resulting contract. Staff shall also be subject to drug and alcohol testing when reasons exist to suspect the use of drugs and alcohol while on duty that impair judgment, performance, operation of vehicles and the safety of Veteran patients, VA employees, visitors, volunteers and other screener staff. Upon request, the contractor shall be expected to provide evidence or copy of the results of employee drug and alcohol testing. *The PPD Skin Test and Annual Flu Shots can be provided and administered by the VA hospital at Mt. Home VAMC. 4. Monthly Invoice Requirement: The invoice shall be itemized to include the information listed below. Incomplete and incorrect invoices shall be returned for correction. (a) Invoice Number and Date date must match date turned in (b) Contract Number (c) Purchase Order/Obligation Number (d) Line item from Schedule of Service associated with each charge and detail (e) Net Payment Due (f) The specific period/dates the invoice is issued to cover. (g) DUNS number (h) EFT information (i) Remittance address (j) Taxpayer identification number (TIN) (k) Copies of each employee time sheet for that invoice period Contractor shall submit original invoice with pre-printed company name. Contractor shall provide a copy of timesheets for employees working during that invoice period with the invoice. For the Government invoice/billing address information, reference Contract Administration Data section. Vendor will not be required to provide a tent, flooring, HVAC, Lighting or Heater. The Mt. Home VAMC will provide these supplies, if needed. FAR Clause 52.223-99 Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors. (OCT 2021) (DEVIATION) is hereby incorporated into this contract. The complete text is as follows: VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors as shown below is hereby incorporated into the scope of work for the subject contract. VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. SCREENER INSTRUCTIONS James H. Quillen VA Medical Center (JHQVAMC) at Mountain Home is currently not allowing visitors to any of the inpatient wards, Intensive Care Unit (ICU) or Community Living Center (CLC) unless an exception has been made. Any non-patient entering the facility must be accompanying a Veteran to an appointment and must be screened before entering. Employees do not need to be screened at this time but must show their Personal Identity Verification (PIV) badge or have a parking decal on the front of their vehicle. If they do not have a PIV badge or decal, they must be screened. Screening: Ask if they have a patient appointment. If response is no, ask why they are here. If it is determined they are here as a surveyor or from an outside organization to perform an inspection (The Joint Commission [TJC], Occupational Safety and Health Administration [OSHA], Office of Inspector General [OIG], or other surveying agencies), treat them as you would a patient or visitor by completing the screening questions below, direct accordingly, then immediately notify Dr. Harold Dillon, (extension 3617), Pam Smith (extension 7306) or Sherry Williams (extension 7636) of their presence and where they are reporting. If response is yes, complete screening questions below. Patients who respond positive for any of these symptoms will be referred to the medical screening tents for additional screening and testing as usual. New/worsening Shortness of Breath (SOB)? New/worsening cough? Congestion or runny nose? Sore throat? Fever? Known exposure to a person diagnosed with COVID-19? Fatigue? Muscle or body aches? Headache? New loss of taste or smell? Nausea or vomiting? Diarrhea? Are you currently waiting on the results of a COVID test? Patients who Respond Positive or YES to the Screening Questions: 1. Provide a mask to all occupants in the vehicle. 2. Explain to the individuals that they must complete a secondary screening before they can proceed to their appointment. 3. Direct them to the large drive-through tents for screening. Patients who Respond Negative or NO to the Screening Questions: 1. Hand a colored dot sticker to the Veteran and caregiver and instruct them to place the sticker on their upper right chest. (Supplies will be provided by the VA) Write the date (for example 03/26) and your initials clearly on the sticker; provider a sticker for the Veteran and one caregiver only. If more than one caregiver is with the Veteran, explain that JHQVAMC policy is that only one caregiver will be allowed to accompany Veteran to an appointment. 2. Release them to proceed to clinic appointment. COVID VACCINE: All Veterans arriving for their COVID vaccination, once screened negative, should be directed to the COVID Immunization Clinic in building 160, second floor atrium (where Starbucks and Phlebotomy are located). All COVID vaccinations are now administered in building160. Screening Tents: The purpose of the small screening tent is to direct patients with symptoms consistent with COVID-19 to the larger drive-thru screening tents. Screening questions are listed in the Screening section on page one above. Remember, keep all people in their vehicles. Ask all vehicle occupants the screening questions. Do not touch the vehicle or open the door. *****There are signs along the road in the screening line stating if a patient is in distress, they should honk their horn, flash their lights, etc., to indicate they are in distress. Contact police (ext 7197) and move traffic to allow that vehicle to continue to the Emergency Department (ED).****** ****If they refuse to go through screening, explain that they cannot proceed unless they go through the screening. If they do not comply, contact police (extension 7197) and provide a description of the vehicle.**** Screening at the Atrium and Other Entrances (except the Community Living Center [CLC)] Building 162): The purpose of this screening station is to screen anyone who may circumvent being screened in vehicles (Veterans who may walk on campus or take public transportation). The ED has its own screening process. Screen anyone who does not have a colored sticker or PIV badge according to Screening on page one. 1. Give them a mask. 2. Explain to individuals who respond positively that they must complete a secondary screening before they can proceed to their appointment. 3. Call screener staff driving carts to take them to the large drive-thru tent for screening when they respond positively to any of the screening questions. Screening at the CLC: If you are positioned at the CLC entrance, the individual entering must be on the approved CLC list and they must still be screened. If they screen positive give them a mask, ask them to be seated away from any other people and call Jennifer Baker (extension 7361), Becky Harmon (extension 2841), Tammy Edwards (extension 2824), or Dr. Valerie Wilson (extension 2030). Main Gates: The purpose of traffic control at this point is to queue patients for screening and allow for ED traffic and staff traffic (including ETSU and cemetery traffic) to flow with as little congestion as possible. 1. Look for the employee sticker on the front, top, center of the windshield. If a sticker is present (or if the employee presents a PIV badge): a. Wave the vehicle to the right or left at the Main Gate (Lamont/Veterans Way) b. Wave the vehicle to the left at the ETSU gate (Mountain Home Way, Railroad Tracks) 2. If there is no employee sticker/PIV badge: Stop and ask if the occupant is trying to get to the ED. If the occupant says yes, direct them to the left at both gates (the goal is to keep the ED patients out of the screening lines; they will be screened upon entering the ED). If they say no direct them to the screening tents. **Vehicles will not be stopped on the railroad tracks. VA police will be available to help if traffic becomes too congested. Limited Access Gates: If you are positioned at the Lamont Street gate entrance near the CLC, only allow delivery trucks, emergency vehicles, city sanitation trucks, and the Johnson City Transit vehicles to enter. This gate is the primary exit gate for all vehicles. No screening at this gate. If you are positioned at the Johnson City Medical Center gate, do not allow vehicles to enter or exit. Open the gate for emergency vehicles only. No screening at this gate. If you are positioned at the Gold Star Boulevard gate, the gate will be open from 5:30 a.m. to 8:30 a.m., and then remain closed. 1. Look for the employee sticker on the front, top, center of the windshield. If a sticker is present (or if the employee presents a PIV badge): Wave the vehicles straight ahead from the Gold Star Boulevard gate. 2. If there is no employee sticker or PIV badge: Stop the vehicle and ask if the occupant is trying to get to the ED. If the occupant says yes, direct them to the left (the goal is to keep the ED patients out of the screening lines; they will be screened upon entering the ED). If they say no, direct them to the screening tent at Veterans Way and Second Street or Lake Drive from ETSU. Screener drivers may be asked to pick up an individual from the main atrium to take them to a large drive-thru tent. For all patients exiting their vehicles, check the patient for a colored sticker and provide patient a ride to the Main Entrance, building 200 near the ED. If you are going to transport a patient wearing a mask, use the designated shuttles only to transport positive screened patients. If you are escorting a patient in a wheelchair to the main entrance from a large screening tent (parking lot O) by pushing a wheelchair, you should wear a mask and face shield. Wear gloves; wipe the wheelchair down with a purple or orange top Santi-cloth or hydrogen peroxide disinfectant spray. ESTIMATED QUANTITIES: The total estimated number of hours over the one-year contract shall not exceed 85,000.00 hours. The guaranteed minimum contract amount for the IDIQ contract for the full contract term is 5,000 hours and the maximum contract amounts shall not exceed 85,000.00 hours. B.2.2. ORDERS: Orders: This is a Firm Fixed Price (FFP) Indefinite Delivery/Indefinite Quantity (IDIQ) contract (see FAR 52.216-22). Task Orders shall be placed against this IDIQ contract at the time of contract award on an as needed basis thereafter. B.2.3. ORDERING PROCEDURES: a. This contract type allows the Veteran Affairs Network Contracting Office (NCO 9) contracting officers to place orders with the Contractors on an as needed basis. In accordance with the Federal Acquisition Regulation (FAR) Clauses 52.216-18, Ordering. NCO 9 will obligate the funding and order the services by issuing a task order.
