Description

Contract Title. Sam Pro 2.0 Acoustic Medicine Device. Background. The Finger Lakes Healthcare System Geriatric Department is in need of 10 Brand name only Sam Pro 2.0 Acoustic Medicine Devices capable of performing pain management and soft tissue healing to reduce chronic pain for our Veterans. Currently the Finger Lakes Healthcare System has multiple devices with components and consumables approved through the Clinical Product Review Committee that have IFCAPS enterted in to the expendable inventory. Five to be shipped to the Canandaigua VA Medical Center 400 Fort Hill Avenue, Canandaigua NY 14424. (528-22-1-6085-0020) Five to be shipped to the Bath VA Medical Center 76 Veterans Avenue Warehouse Bldg 103, Bath NY 14810. (528-22-1-6085-0021) Scope. The Finger Lakes Healthcare System has a requirement to procrure 10 each brand name only Sam Pro 2.0 Acoustic Medicine Devices, item number 1075906. Currently the Finger Lakes Healthcare System has multiple devices with components and consumables approved through the Clinical Product Review Committee that have IFCAPS enterted in to the expendable inventory. Five to be shipped to the Bath VA Medical Center 76 Veterans Avenue Warehouse Bldg 103, Bath NY 14810. Five to be shipped to the Canandaigua VA Medical Center 400 Fort Hill Avenue, Canandaigua NY 14424. Specifications: Ship to Bath Facility: 5 Each Sam Pro 2.0 Acoustic Medicine Devices.
