This is a SOURCES SOUGHT NOTICE/REQUEST FOR INFORMATION ONLY. Responses to this notice will be used for information and planning purposes and this notice is being posted to Contract Opportunities at (https://beta.sam.gov). No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The Department of Veterans Affairs Medical Center, Veterans Health Care System of the Ozarks, Fayetteville, Arkansas has a requirement for onsite Anatomical and Clinical Pathology Services to eligible beneficiaries. The physician shall provide comprehensive pathology services covering Anatomical and Clinical Pathology (including but not limited to: Surgical Pathology, Cytology, and Autopsy Service). In addition, this includes all work associated with interpretations, blood smears, surgical gross, surgical pathology, consultation slides, frozen sections, and autopsies/autopsy blocks. The physician shall also provide services covering Clinical Pathology to include On Call for Blood Bank. The position is for a Board-Certified or Board Eligible Anatomical or Clinical Pathologist. Basic Life Support (BLS) is required. 2,080 Hrs. for 1 Year Base Period and four additional 1-year ordering periods. Place of Performance: Contractor shall furnish services at Veterans Health Care Systems of the Ozarks, 1100 N. College Ave., Fayetteville, AR 72703 Offeror s response to this Notice shall be limited to 10 pages and shall include the following information: Offeror s company name, DUNS, Federal Tax ID, address, point of contact, title, phone number, and email address Offeror s interest in bidding on the solicitation when it is issued whether it be issued to VA FSS/GSA Schedule holders via e-BUY or on SAM Contract Opportunities (https://sam.gov). Please provide VA GSA schedule number if you have one that covers Pathologists at Fayetteville, AR. Offeror s capability to meet this requirement and demonstrate how best the VA could set up pricing schedule to allow for better offers to include break down of expenses that your company would submit billing for. Offeror s type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hub-zone, 8(a), Woman Owned, Small Disadvantaged, etc. The NAICS code is 621111, Offices of Physicians (except Mental Health Specialists) (size standard of $12 million). Responses must be received by no later than 3 pm Central Daylight Time, 20 December 2021. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought notice.
