Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is RFQ-2022-60354-SK and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information including a list of line item number(s) and items, quantities, units of measure, and options, if applicable.� CLIN 0001:����� ��������������������������������������������������������������������������������� Pipette Maintenance The selected vendor will provide cleaning and calibration of 1000 pipettes during a one-week onsite visit.� This service should include cleaning, calibration, and minor repair on most major brands and types of pipettes. Brands include, but are not limited to, Rainin, Eppendorf, Oxford, Labsystems, Finn, Titertek, Biohit, Wheaton, MLA, Matrix, Gilson, Denville, Nichiryo, and Tranferpette. Multi-channel and single-channel pipettes will need to be serviced.� TOTAL PRICE: Your quote shall include the following: Total Price for CLIN 0001 Capabilities in response to minimum qualifications Period of Performance: 01/01/2022 to 01/31/2022 NIOSH Morgantown 1000 Frederick Lane, Morgantown, WV 26505 The quote format is at the discretion of the offeror. QUOTERS ARE ADVISED AN AWARD MAY BE MADE WITHOUT DISCUSSION OR ANY CONTACT CONCERNING THE QUOTES RECEIVED. Quoters should not assume they will be contacted, or afforded the opportunity to qualify, discuss, or revise their quotes. Therefore, the initial quote should contain the quoter's best terms from a technical and price standpoint. All quotes must include the full name, address, and DUNS number of the business to be named on the purchase order as well as the full name, phone number, and email address of the person who can accept the final purchase order. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2021-07 (August 11, 2021.) This acquisition is set aside for Small Businesses under North American Industry Classification System (NAICS) code 811219 and small business size standard of $22 Million. A Firm-Fixed Price purchase order will be issued in writing to the successful offeror. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the System for Award Management. To register apply via the Internet at http://www.sam.gov. Evaluation and award of the purchase will be performed in accordance with applicable sections of FAR Part 12 Commercial Items and FAR Part FAR 13 Simplified Acquisition Procedures. The Offeror must have an active registration with the System for Account Management (SAM) prior to award of the purchase order in accordance with FAR Clause 52.204-7. QUOTES MUST BE MADE IN TWO PARTS: A TECHNICAL PROPOSAL AND A PRICING PROPOSAL: After evaluation of the following technical factors, award will be made to the lowest priced technically acceptable offer. Each offer will be evaluated on a pass/fail basis, so offerors are cautioned that they should submit responses in sufficient detail to enable the government to evaluate their capabilities and experience. Minimum qualifications: In order to be technically acceptable, the technical proposal must demonstrate that the services offered include: 1. Pipette disassembly and interior and exterior cleaning; 2. New seal installation and/or relubrication according to manufacturer�s recommendations; 3. Pipette reassembly; 4. Recalibration at high, mid and low volumes; 5. Recording of 4 handwritten readings at the high volume; 6. Application of sticker with date of service and date due for next service; 7. Six Month to One Year Service Warranty; 8. ISO17025 certification documentation. 9. Minor repairs should also be performed, as needed, providing the necessary replacement components are available.
