Description
This Request for Quote (RFQ) is for One (1) Non-Personal Service Contract for Intensive Care Unit Registered Nurse on ""As Needed Basis"" to provide uninterrupted patient care services to be performed in the Intensive Care Unit at Northern Navajo Medical Center (NNMC) located in Shiprock, New Mexico. The tasks to be performed shall be performed in accordance with the attached Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP). Period of Performance:� Date of award/selection through 12 months. The assignment are through the stated period of performance or until the position is filled with a permanent hire. In such instances, the provider will be released from the contract. WORK SHIFT: The shifts are 12 hour rotational (day or night) including weekends and Government Holidays. The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The Supervisor at IHS shall determine specific tour of duty. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated. Housing: No Government Housing available. Facility Location: Northern Navajo Medical Center is located in the beautiful high-desert climate of the Four Corners area - where the four states of Arizona, New Mexico, Colorado, and Utah adjoin. Northern Navajo Medical Center provides: care services including family medicine, internal medicine, mental health, physical therapy, optometry, dental, social services, pharmacy, x-ray, laboratory, public health nursing, diabetes education, health promotion/disease prevention and outpatient rehabilitation services along with benefits coordination. Recreation opportunities exist such as camping, fishing, hiking, biking, backpacking, snowshoeing, snowboarding, cross country skiing, and alpine skiing. Telluride, Durango, Moab, Canyonlands, Grand Gulch, the Colorado River, the San Juan River, several large lakes, and Colorado Wilderness areas are just short drives away. CONDITION OF EMPLOYMENT: Type of Citizenship: U.S. Citizenship is required. Background Investigation: All qualified referred candidates will undergo Fingerprinting process. Fingerprint Card: FBI FD-258 (provided by the Federal Government): Fingerprinting MUST be cleared and approved prior to contact assignment. Contract assignment(s) are conditional on a cleared background check. OPM requires Electronic Questionnaire for Investigation Processing (e-QIP) for registered nursing services exceeding One Hundred Twenty (120) days. Electronic Questionnaire for Investigation Processing (e-QIP): Executive Orders 10450, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM) system. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant subfactors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. INITIAL APPOINTMENTS REQUIREMENTS: Education: Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Factor 1: Agency's Past Performance Subfactor: Number of similar contracts Subfactor: Timeliness of providing candidate(s) Factor 2: Registered Nurse Certifications (Mandatory) Requirements (Refer to PWS 6.3) Subfactor: A valid and unrestricted Nursing License. Subfactor: A current Basic Life Support (BLS) Certification with American Heart Association. Subfactor: A current Advance Cardiac Life Support (ACLS) Certification with American Heart Association. Subfactor:� Preferably have TNCC and Pediatric Advance Life Support (PALS) Certification with American Heart Association Medical Evaluation: The contractor shall provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others. Immunization: Contractor shall provide evidence of the following: -������������� MMR (measles, Mumps, and Rubella), -������������� Immunity to Hepatitis A and B -������������� Flu Shot -������������� History of chicken pox disease of positive titer -������������� Tetanus Diphtheria (Td) within the last 5 years -������������� Documentation of receiving a TB skin test (PPD) within the past 12 months with documentation of follow-up for a positive test. -������������� A signed delination of the Hepatitis B vaccination series shall be accepted. ***A copy of the document shall be submitted to the Employee Nurse upon arrival at the hospital before providing service. Immunization: Contractor shall provide evidence of current immunization documentation for Flu Shot, Immunity to Hepatitis B series, and History of chicken pox (varicella) disease or positive titer, Tetanus Diphtheria (Td) within the last 5 years. Documentation of receiving a TB Mantoux skin test (PPD) within the past 12 months with documentation of follow-up for positive test.� Factor 3:� Registered Nurse Availability Subfactor: Upon fingerprinting clearance, contractor shall be available as soon as possible to start. Subfactor: Registered Nurse shall be flexible to allow for short term (13-weeks), long term (1-year) and, where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. Subfactor: The contractor shall rotate into other duty sections as needed to support patient care. Factor 4: Professional Nursing Skills: Subfactor: RN must have a minimum of 3 years Critical Care Nursing experience and be able to work independently. Subfactor: The nurse needs to possess a strong basic and advance critical care nursing knowledge, clinical skill and competency. The nurse needs to be competent in the care of a comprehensive, acute and chronic critically-ill adults ranging from young adulthood to geriatrics. This may include caring for patients with postpartum complications. Subfactor: The RN needs to know the care of patients, who require the use of invasive hemodynamic monitoring (transducing central venous pressures, arterial lines and abdominal pressures); titration of vasopressors, anti-arrhythmic, sedatives, paralytics, etc.; care of post-operative patients, those patients requiring blood product administration, and etc. The RN needs to be competent to assist the medical provider with bedside procedures (central line placement, arterial line placement, lumbar puncture, thoracentesis, paracentesis, etc.). Subfactor:� Anatomy & Physiology of all age groups: young adulthood to geriatrics. Subfactor:� Proficient in IV cannulation/blood draws of all age groups. Subfactor: Assess all ages and communicates abnormal signs and symptoms to Medical Provider appropriately. The RN needs to be able to problem solve, critically think, and make appropriate and immediate clinical judgment and decisions. Subfactor: Ability to work with Electronic Health Record & Barcode Medication Administration Scanners. Subfactor: Ability to float to Emergency Room Department and/or PACU upon completion of a Float Competency Skills Checklist. This is merely a preference and not mandatory for this position. Subfactor: The RN primary job will be within, the ICU, but they will also need to be able to cover the INFUSION CLINIC.� The infusion clinic requires nurses to be able to infuse basic IV drugs including antibiotics, blood, and other IV piggy backs. Subfactor: Ability & knowledge to learn the different types of equipment. Subfactor: Charge Nurse Duties: Responsibility and Expectations Subfactor: Orientating of other nurses responsibiliies Equipment skills: - VIASYS ventilators Nihon-Khoden Cardiac monitors and telemetries Level 1 Rapid Infuser Alaris IV and PCA pumps SCDs (Sequential Compression Devices) Kangaroo feeding pumps LINET (5 new beds) Pressure Monitoring for hemodynamic purposes **** Licensure: Must possess and maintain current State licensure while employed with Indian Health Service (IHS). Requirements: 1. Quotation in the line items above. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The Attach Wage Determination Service Contract Act applies. 2. Past Performance: Provide experience of comparable scope and complexity in providing services within the past three (3) years of operation. 3. Qualifications: Provide complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information, Professional Nursing Skills and all relative documents for proposed candidate. Refer to PWS Section 5.0 & 6.0). 4. Availability of candidate(s). 5. Required to float to other units on an as needed basis. NOTE: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers.
