- This request for information (RFI) is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI.� Interested parties responding to this RFI shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. Responses are due by December 16, 2021 at 4:30pm MST. It is anticipated that a solicitation may be issued in January 2022. If issued, award of a subsequent contract is contemplated by January 2022. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. STATEMENT OF WORK Purpose: The Phoenix VA Health Care System (Agency) requires a service agreement to install and provide maintenance on its Medical Reconciliation Tool, reference paragraph #3 for details. Background: The Agency has a need for a Medical Reconciliation Tool.� The Agency doesn�t have anyone who is certified or trained to repair and perform preventive maintenance on this software.� Therefore, the Agency needs to hire someone who is trained and certified to service the tissue processor to perform preventive maintenance and repairs. The Medication Reconciliation Tool (MedRec tool) will need to work by combining all medications from multiple sources into a single easy to manage, master list. These medications need to be logically ordered so that the user can efficiently and accurately reconcile medications at both admission and discharge. The tool should allow the provider to generate patient-friendly lists emphasizing new, changed, or discontinued medications. The tool will be used in the Inpatient Clinical and the Emergency Departments to generate a pre-appointment medication list that allows the patient to play a role in providing an accurate medication list prior to their hospitalization visit. The MedRec tool will need to fully integrate with VistA/Computerized Patient Record System (CPRS). The tool will need to assist with the VA-specific workflow for all health providers. The ability for the tool to interface with VistA/CPRS and incorporate VA-specific provider workflow is vital as the end product list will need to pull and push information between VistA/CPRS and the software tool in order to promote patient safety. The MedRec tool must be able to aggregate medications from multiple VA sites (i.e., other VA medical centers and outpatient clinics, inpatient medications, outpatient medications, suspended/inactive/ expired medications, non-VA medications, as well as remote medications found in the separate program VistA Web). The tool needs to be able to construct an intuitive single display of medications which allows for a complete, efficient, and rational review of a patient�s entire prescription medication history and is the basis for perfect reconciliation of these medications. This tool must allow physicians to have a complete picture of the patient�s medical history so they can make more informed decisions on future treatment. The tool needs to be both physician-centric and patient-centric with the ability to generate a patient-friendly medication list, that clearly specifies new, changed, or discontinued medications, which can be given to the patient if desired. Prior to possessing this capability, pharmacists and physicians using CPRS had to reconcile medications by frequently relying on incongruent medication lists, which increased the potential of committing a significant medication error and jeopardizing patient safety. 3.
