- *** NOTE� It was just brought to our attention that the attachment��Amendment 6 Revision 5 -Attachment 3 - Price Matrix-WBAMC- 1 Dec 21 TAB 0006 - Bldg. 18511E-East Clinic�needs Column G for�Max SF for CLIN Years 2-5 updated from 203,191 to 202,390. Please make this update to�Column G for Years 2-5 on TAB 0006. There should be no other changes *** Amendment 0006. See attached amendment. The proposal due date of 3 December 2021 has been revisied to 6 December 2021, 12:00 p.m. Central Standard Time.�The purpose of this amendment is to correct square footage subtotals for contract line item numbers, 0006 (Bldg.18511E), 0007 (Bldg.18511W), 0009 (Bldg. 1026), 0014 (Bldg. 11282), 0017 (Bldg. 11335) and 0018 (Bldg. 1363), which then resulted in a change (decrease) to the overal square footage from 1,367,990 to 1,367,605. Exhibit C - Summary of Bldg. Sq. Ft/Type Service (New-Revision 2), Exhibit D-Detail, Building/Rooms/Sq Ft/Type Service (New -Revision 2), Attachment 3, Price Matrix (New -Revision 6), Attachment 4, Staffing Charts (New -Revision 3) and Performance Work Statement paragraph 1.4 have all been revised as a result. The point of contact for this amendment is Ms. Quinella M. Purks at quinella.m.purks.civ@mail.mil. All other terms and conditions remain the same. There was a glitch�in the system, there were no changes made to the Addendum to 52.212-2. However, the amendment states that it was revised but it was not. A Conformed Copy is also included as a part of this amendment. Amendment 0005.� See attached amendment.��The proposal due date of 3 December 2021, 12:00 p.m. Central Standard Time remains.��The purpose of this amendment is delete Bldg. 2496, Soldier Family Medical Clinic from Attachment 4 (Base, Year 2 and Years 3 -5), Type II services, update Attachment 3, Price Matrix with Revision 4. The updated Attachment 3, Price Matrix revises the max SF for Bldg. 18511E from 196,423 to 203,191.� All other terms and conditions remain the same. Amendment 0004 - See attached amendment.��The proposal due date of 3 December 2021, 12:00 p.m. Central Standard Time remains.� The purpose of this amendment is to correct formula errors on Attachment 3, Price Matrix.��The updated Attachment 3, Price Matrix is now Revision 3. CLIN 0007, Bldg 18511W � West, Max SF cells G10, G19, G28, and G37 have been corrected to indicate max SF of 222,663. CLIN 0038, Bldg L10735 � Sum of Unit Prices for cells D27, D36, and D45 have been corrected so that it sums all of the applicable fields.� All other terms and conditions remain the same. Amendment 0003 - See attached amendment.� The proposal due date of 3 December 2021, 12:00 p.m. Central Standard Time remains.� The purpose of this amendment is to update Attachment 1, MTF Description_Workload Data, post a second set of Questions and Answers and provide an updated Attachment 3, Price Matrix, Revision 2, which corrects an error found on the CLIN Summary Tab.� CLIN 0041 was linked to the wrong sheet.� It has been updated to be linked to the Linen Services sheet instead of the Phase-In Sheet.� All other terms and conditions remain the same. Amendment 0002 - See attached amendment.� The proposal due date of 3 December 2021, 12:00 p.m. Central Standard Time remains.� Please read amendment carefully.� Exibits, C, D, E, G, H, K, Attachments 1, 3 and 4 have been revised and other minor changes to the Performance Work Statement and Addendum to 52.212-1.� Additionally, Attachment 5 is attached with government responses to contractor questions. A conformed copy of the solicitation is also attached to this posting. ****NOTE - DO NOT REQUEST DOD SAFE CODES DURING THE WEEK OF THANKSGIVING.� REQUEST CODES ON 30 NOVEMBER 2021, 1 AND 2 DECEMBER 2021 ONLY. All other terms and conditions remain the same. Amendment 0001 - The proposal due date for this requirement has been extended from 16 November 2021 to 3 December 2021, 12:00 p.m. Central Standard Time.� All other terms and conditions remain the same.� See the attached Amendment.� Additionally, another amendment will be forthcoming o/a 12 November 2021 with revised Exhibits and Attachments.
