- Intent to Sole Source The Department of Veterans Affairs, NCO 21, intends to award a sole source firm fixed price contract to Pu`ulu Lapa`au for Impaired Provider Monitoring Services for the VA Pacific Islands Healthcare System (VAPIHCS). We believe Pu ula Lapa au is the only company in the State of Hawaii that can perform these services. The requirement requires that the vendor provide monitoring services for local healthcare professionals. Monitoring includes weekly in-person visits with impaired physicians, coordinating with local healthcare resources for the help of the impaired physicians, direct contact and outreach to the impaired physician. A draft of the performance work statement is attached. If you believe that your company is capable of providing these services in accordance with the draft performance work statement, please provide your information, answers to the questions below and capability by responding to angela.gerlitzki@va.gov no later than December 15, 2021 10 a.m. HST referencing the following number in the subject of the email 36C26122Q0123. The applicable NAICS Code is 624190 and the size standard is $12 Million. This notice of intent is not a request for competitive quotes. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government. Capability Statement Please answer the following questions when responding: What is your company s Socioeconomic Status? (ex: small business, large business, SDVOSB, etc.) Provide DUNS and/or CAGE Code. Anticipated Teaming Arrangements (if any). If plan on subcontracting, demonstrate that you will meet the limitations on subcontracting per FAR 52.219-14 for Small Businesses; VAAR 852.219-74 for SDVOSB/VOSB. GSA Contract Number, if applicable. Pricing per the Performance Work Statement. Do you provide monitoring services? Describe. Do you make contact with impaired providers directly? Describe. Do you coordinate the referral and management of providers through the care/treatment phase? Describe. Describe educational opportunities for the health of healthcare professionals provided by your company. Provide a reference in which you have performed similar services (in size and scope). If you are not located within the State of Hawaii, please describe how you intend to perform these services. Are you able to comply with Joint Commission requirement MS.11.01.01?
