Description
STATEMENT OF WORK (SOW) Project # 561-19-160 Upgrade Prosthetics Lab 9/22/2021 1. Contract description. Provide Construction services for project 561-19-160 Upgrade Prosthetics at the VA East Orange, NJ Medical Center. 2. Background. The current Prosthetics Lab is antiquated and broken up into separate sections. This flow is inefficient and requires updating to enhance the patient experience. The current space in broken up by the telephone operator suite which will be relocated. This project will also address any outstanding Facility Conditions Assessment (FCA) deficiencies located throughout the space. 3. Scope. Contractor shall completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials to perform work for VA Project 561-19-160 Upgrade Prosthetics Lab as required and defined by the Architecture and Engineering (AE) drawings and the specifications. 4. Specific Tasks. A. The scope of work shall include but not be limited to the following main items: 1. Demolition: i. Architectural- Demolition of interior walls, finishes, acoustical ceiling panels and grids and other components as noted on the Architectural Drawings ii. Mechanical -Demolition of ducts and supports and other components as noted on the Mechanical Drawings iii. Plumbing- Demolition of sinks, pipes and other components as noted on the plumbing drawings. iv. Electrical- Demolition of electrical and communication components as noted on the electrical Drawings. v. Fire Protection- Demolition/modification of existing fire protection components as noted on the Fire Protection Drawings. vi. Abatement of hazardous materials. 2. New Work: i. Architectural- Provide and install new interior walls, doors, acoustical ceiling grids and panels, gypsum wall board soffits, wall protection, handrails, and interior finishes. ii. Mechanical- Reconfigure existing system and provide new equipment to serve renovated areas per the mechanical Drawings iii. Plumbing- reconfigure existing systems and provide new equipment and piping to serve the renovated area per the Plumbing drawings. iv. Electrical- Reconfigure the existing systems and provide new equipment nd wiring to serve the renovated area per the Electrica Drawings. 3. Project Area is roughly 4,695sqft, and include most if of the A wing on A level building 1. 4. Work includes but is not limited to the following items in Stairwell A-S7 i. Removal of carpet on the floor and replace with specified flooring on the drawings. ii. Removal of doors to rooms A-103G and A-E-102 iii. New door installation in room A-103J into room A-103G iv. New opening (as specified on drawings) from room A-E-102D to A-E-102. B. Statement of Bid Item (s): 1. Work includes demolition, hazardous material removal, general construction, alterations, mechanical construction, electrical construction, plumbing construction, and fire protection construction, and certain other items for the renovation of existing space. 2. Alternate no. 1: Delete Pass Window and Fire Rated Coiling Counter Door and associated electrical work between Reception A-701 and A-70. Continue Fire-Rated Wall Barrier as required. 5. Performance Monitoring (if applicable). Work shall be coordinated with facility thru the Contracting Office Representative (COR). COR will monitor the contractor work performance on daily basis and the contractor is required to submit daily s progress report to the COR. 6. Security Requirements. General Contractor s employees shall not enter the project site without appropriate security badge. They may also be subject to inspection of their personal effects when entering or leaving the project site. No photography of VA premises is allowed without written permission of the Contracting Officer except as specifically required by the scope of the work. VA reserves the right to close or shut down the project site and order General Contractor s employees off the premises in the event of a national emergency. The General Contractor may return to the site only with the written approval of the Contracting Officer. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). N/A 8. Other Pertinent Information or Special Considerations. a. Identification of Possible Follow-on Work. Not Used b. Identification of Potential Conflicts of Interest (COI). Not Used e. Inspection and Acceptance Criteria. The COTR is responsible for certifying that the work done under the contract is performed to time and standard. They are also responsible to assure the inspection and acceptance of products provided incidental to services. 9. Risk Control Contractor shall take all measures for safety & infection control and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Centers operations will not be hindered. Contractor shall permit access to Department of Veterans Affairs personnel and patients through other construction areas which serve as routes of access to such affected areas and equipment. Coordinate alteration work in areas occupied by Department of Veterans Affairs so that Medical Center operations will continue during the construction period. 10. Place of Performance. 385 Treamont Ave, building 1 A level A-Wing. 11. Period of Performance. Contractor to start immediately after receiving the NTP, estimate completion time is 180 calendar days.
