Description: The Defense Health Agency (DHA) modified the current contract to OPTUMSERVE (Previously Known as �Logistics Health, Inc.� on a non-competitive basis for a 12 month extension (6 months base and two 3 months each options) in order to provide medical and dental health services to the Reserve Health Readiness Program. The current contract HT0011-19-D-0002 was originally awarded for two (2) six (6) month option periods and subsequently modified (P00007) to extend the period of performance for another 12 months. Including this modification, the total contract period of performance will be 4 years
