- Amendment 0001 to Request for Quotation (RFQ) 36C26322Q0132 is to answer questions, establish a site-visit, correct the start date, and extend the response due date. 1 The current contract for elevator maintenance is held by Kone, Inc. under GSA task order GS-06F-0002N VA263-16-F-0938; 2 There are currently no third-party elevator inspection violations for any elevators located at the Omaha and Grand Island, NE facilities; 3 A site-visit is scheduled for the Omaha facility on 12/16/2021 at 10:00am CST and at the Grand Island facility on 12/17/2021 at 10:00am CST. It is requested that all potential contractors planning on coming to the site-visit RSVP with the facilities Engineer, Roy Hurd, via email at roy.hurd@va.gov. Phone: 402-995-4450 Failure to come to the site-visit will result in accepting all equipment as-is should the vendor win the contract. 4 The period of performance is hereby corrected to: Base Year: 03/01/2022 02/28/2023 Option Year 1: 03/01/2023 02/29/2024 Option Year 2: 03/01/2024 02/28/2025 Option Year 3: 03/01/2025 02/28/2026 Option Year 4: 03/01/2026 02/28/2027 4 The due date for all responses is now 12/22/2021 at 10:00am CST.
