- 36C24922Q0014 Page 1 of Page 1 of Page 1 of Page 1 of 1. The purpose of this amendment is to remove the below FAR Clause 52.223-99 from the Statement of work. FAR Clause 52.223-99 Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors. (OCT 2021) (DEVIATION) is hereby incorporated into this contract. The complete text is as follows: VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors as shown below is hereby incorporated into the scope of work for the subject contract. VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. Page 1 of Page 1 of Page 1 of Page 1 of 2. The purpose of this amendment is to incorporate the following answers to questions submitted by interested vendors: 1. On Page 15 it states Supervision on the schedule, does that refer to a building or a person? Person 2. How many supervisors are you requesting for this contract? 1 supervisor, 1 lead screener 3. What is a lead Screener and what do they do? Also, how many lead screeners are you requesting? The lead screener is the assistant supervisor. They cover the supervisor when the supervisor is out to endure there is coverage at each screening station and helps with scheduling. 4. On page 17, it says 6 am to 8:30 am, is this a typo and is it supposed to say 6 am to 8:30 PM? No, we only need a screener in this area for 2 hours. After 8:30am the gate is closed. The only reason the gate is open is for employees arriving to campus. Since the majority of employees come in between 6a-8:30am they use this entrance to keep clutter minimal at the other entrances for patients. 5. Has the JHQVAMC procured these services previously? Yes. 6. If there is an incumbent contractor for this requirement, can the government please provide the contractors name and contract number? 36C24921D0056 36C24921N0537 Community Pastor Care, LLC 7. Can the government confirm if this is a personal or non-personal services contract? Personal/Service Contract 8. Does the government anticipate a single or multiple awards? Single award 9. SAM.gov has two versions of the solicitation uploaded (one in word and PDF). If there are any differences between the two, which version should the offeror utilize? PDF and Word documents is the same. 10. Referencing Page 8: Will the government provide the masks that are to be distributed to visitors? Yes 11. Referencing Page 8: Can uniforms have logos pinned to them, similar to a large name tag or button, in order to be compliant with the uniform standards? Yes, we prefer all screeners to have the same color uniform and they are issued a PIV badge by VA 12. Referencing Page 8: Can the contractor have the logo on the vest only as opposed to all uniform items? All screeners are required to wear a reflector vest on top of the uniform. A logo isn t needed as much as having the same color uniform and they are issued a PIV badge by VA 13. Referencing Page 8: Does the ""brightly colored"" requirement refer predominantly to the vest or are brightly colored uniforms referring to all clothing item requirements? The Vest. Current screeners wear maroon uniforms. 14. Referencing Page 8: Does bright white or other light colors qualify as ""bright"" or does ""brightly colored"" refer to neon only? The reflector vest needs to have bright, safety yellow colors on them. Bright white or other light colors are fine for the uniform only as long as a reflector vest is worn on top of it. 15. Referencing Page 8: Can the contractor substitute other ANSI certified vests for this requirement? Yes 16. Referencing Page 9: What kind of office supplies and miscellaneous items will the contractor be expected to provide? Pens and paper. Laptop or an electronic device to receive emails from the COR and project manager. 17. Referencing Page 9: Does the government have any specifications regarding the amount of personnel required for the on-call pool? Historically on-call employee has not been utilized, but 1-2 employees on call will suffice to cover staffing shortage due to call-ins. 18. Referencing page 14: The PWS mentions screener drivers . Is the offeror expected to provide these services? If so, will the vehicles the screeners be driving the property of the government, or will they be expected to utilize personal vehicles? Screener drivers are screeners that drive golf carts around the campus for patient pick up and drop off to/from the parking lots, buildings and/or COVID testing tent. The Government provides the golf carts and maintenance. 19. Referencing page 15: Will the government provide masks, eye protection, gloves, disinfectant spray, and Santi-cloth for the contract employees to utilize in performance of the PWS? Yes 20. Referencing Page 19. Are there any specific qualification requirements for the Lead and Supervisor positions? Great communication skills 21. Referencing Section D: Can the Government confirm if offerors are required to use the WD amounts listed in section D, or if they have the flexibility to cross walk a different SCA position (such as a customer service representative) to the current requirement? No. Vendor must comply with COVID Screener (Tennessee State wide) listed in Wage Determination. 22. The CLIN references 52,200 hours yet the estimated quantities states 85,000 hours NTE with a guarantee of 5,000 hours is there a reason or days which the workers would be told to leave before the stated working hours? This is a IDIQ contract and RFQ must state minimum and maximum quantities. We do not expect the COVID Screener Contract to stop anytime soon. 23. Do you have a breakdown of the FTE allocation or locations for the potential option years? No. This is for one year contract. 24. Are there any page limits for the response? No 25. Does pricing need to be provided in a separate volume, or can it be located in the same volume as the technical response? See question number 31 26. Are there any special formatting requirements, such as typeface, margins, etc. 27. There is a discrepancy in the date for Govt to receive the quote. SF 1449 indicates Dec 15, and Paragraph 2 of page 51 indicates Dec 17. Question: Will the government please clarify correct date? See Solicitation Amendment 0001. 28. Can the government indicate what traffic safety protocols, if any, are in place to protect the Contractor screeners? Traffic safety street/signs throughout the campus, screeners wear reflector vests. When early morning or night there are streetlights put in place of screening and entrance areas. Each screening area has a radio to contact Police dispatch in case of emergency. 29. Within the COVID SCREENERS BY LOCATION maps the earliest time listed is 5:30am. Therefore, do the M-F operation begin at 5:00am or 5:30am? 5:30am 30. Can the government please indicate the correct number of paid holidays, 10 or 11? 11 days 31. Is the Government asking vendors to submit pricing in accordance with the table provided under Item Information ? Yes. Please submit your quote as shown below.
