Page 1 of 3 This is a Sources Sought notice 36C24422Q0202; this is neither a solicitation nor a promise of future work. The Wilmington VAMC is seeking SDVOSB/VOSB and any small or large company regardless of Socio-economic category that can provide the Brand Name Welch Allyn requirement as spelled out in the Statement of Need (SON) and list of items below. The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have permission to provide and install any systems or software. Are you a manufacturer, dealer, or distributor? New medical equipment will be required, all resellers, dealer, distributors must provide an authorized distributor letter from the Original Equipment Manufacturer (OEM). INTRODUCTION/BACKGROUND: The Wilmington VA Medical Center located at 1601 Kirkwood Highway, is soliciting for a vendor to provide Brand Name Welch Allyn vital sign monitors with the capability of transmitting vital signs from the device to the patient s electronic medical record system used by the VA, the Computerized Patient Record System (CPRS). The vendor shall provide all parts and labor to check in services to confirm correct operation of each device. Vital sign monitors are used in all inpatient, outpatient, and long-term care areas, and are vital devices for sustaining world class of the are medical care. OBJECTIVE: The Department of Veterans Affairs Medical Center, (460) Wilmington, Delaware, hereby referred to as the VAMC Wilmington (460), is seeking the acquisition of Welch Allyn vital sign monitors and accessories that have the functionality to connect to the network with the purpose of downloading vitals signs in the patient s CPRS. This need for this acquisition is determined to be routine and can be awarded within designated Contracting timeframes. To meet the needs of this acquisition, the following salient characteristics or needs must be met, and Offeror shall provide: 13 Welch Allyn Connex 6800 Vital Signs Monitor with Nellcor SpO2, SureTemp Plus Thermometry, SupreBP Non-invasive Blood Pressure, Pulse rate, MAP, Custom Scoring, Nurse Call, Four USB Ports for Accessories, Internal 802.11 a/b/g Wireless Radio; 100-240 V, 50-60 Hz AC, IEC Plug Type-B 13 Welch Allyn Accessory Cable Management Mobile Stand for Connex Vital Signs Monitor 6000 Series; with Storage Bin 13 High Performance Welch Allyn HS-1M 2D Barcode Scanner with Coiled USB Cord MAJOR REQUIREMENTS: The Contractor/ Vendor must complete the following tasks to meet the needs of this acquisition: Coordinating delivery with COR and or facility. Deliver equipment to: Wilmington VA Medical Center (ATTN: Thomas Hill) 1601 Kirkwood Hwy Wilmington, DE 19805 Upon delivery, the contractor/vendor agrees to work with facility personnel in order to: Conduct a joint inventory of goods by material number, quantity, and serial number De-palletize shipment, unbox, and stage products Unbox and assemble roll stands Unbox and assemble wall mounts Attach devices to stand or wall mount Attach accessories to each device (SpO2 cable, NIBP hose, cuff, temperature sensor, power supply, barcode scanner, and quick reference card. Affix customer provided asset tags if required Perform device in-service. Dispose of waste packaging Register device warranty Perform ground continuity checks, leakage current, and power-on tests as required. Unboxing and assembly Disposal of any shipping/packaging materials Assist in the distribution of new devices to the nursing floors as needed to deploy new devices. Configure devices per customer requirements if needed Provide Summary Report The Government and the Contractor understand and agree that the services to be delivered under this contract by the Contractor to the Government are non-personal services and the parties recognize and agree that no employer-employee or master-servant relationships exist or will exist under the contract between the Government and the Contractor s employees. GOVERNMENT FURNISHED INFORMATION OR MATERIAL: This acquisition does not include Government-furnished property (GFP), identify in the Statement of Work all available information on GFP to be provided, including the make, model, bar code number and serial number, if available, of all accountable assets with an acquisition cost of $500 or more. This acquisition does not include Contractor-acquired property (CAP).
