Scope The intent of this acquisition is to acquire specific ophthalmic solutions prepared by a Vendor and shipped to the facility as needed by the Jamaica Plain VAMC Pharmacy. This is a firm-fixed price contract for one year. Quantities can be found in section 5 Deliverables of the PWS. General Requirements Vendor shall prepare and ship Ophthalmic Solution Syringes and droptainers to the facility and/or patients as directed by the Pharmacy representative. Non-Personal Services The Government shall neither supervise contractor employees nor control the method by which the contractor performs the required tasks. Under no circumstances shall the Government assign tasks to, or prepare work schedules for, individual contractor employees. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services, or give the perception of personal services. If the contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the Procuring Contracting Officer (PCO) immediately. Location and Hours of Work Accomplishment of the results contained in this PWS requires work to be performed at the Contractor s facility. Technical: a.       Contractor shall demonstrate their corporate experience and approach to meet all requirements stated in the Performance Work Statement. Contractor shall include all training certificates associated with the services prescribed in the PWS. Subcontractors As a part of the Technical Quote, vendor will identify all subcontractors (including a list of all elements applicable to the utilization of subcontractors) to be involved in providing services to the VAMC. Subcontractor(s) identified in the vendor s technical quote will be the same subcontractor(s) who will actually perform on the project. If, during performance of the contract, it becomes necessary for the vendor to substitute subcontractor(s) for one of those whose qualifications were the basis for evaluation of the offer and award of the contract, the vendor shall submit to the Contracting Officer the same information that was required in the initial quote regarding the proposed substitute subcontractor(s). The Contracting Officer shall have the right to review the qualifications of the proposed substitute subcontractor(s) and to approve or disapprove the substitute subcontractor(s) qualifications.
