Description

Amendment One Dated: 15 December 2021 The purpose of this amendment is to� 1. Answer Questions. 2. Extend the quote due date to 11AM Mountain Time on 20 December 2021 3. Upload the revised performance work statement. 4. Make minor corrections to the combined synopsis/solicitation document to correct CLIN descriptions and due date. 5. Please see revised attachments dated 15 December 2021.� Original Text: This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for Solicitation Number W911RZ-22-Q-0003. The Mission Installation Contracting Command (MICC) - Fort Carson intends to award a Firm-Fixed-Price Contract to a total small business set-aside for the acquisition of Musician Services at Evans Army Community Hospital at Fort Carson, Colorado. This contract is being solicited for base year period of performance (POP): 03 January 2022 through 02 January 2023; with the option years of 03 January 2023 through 02 January 2024; and 03 January 2024 through 02 January 2025. This is a 100% total small business set-aside. The North American Industrial Classification System (NAICS) code for this procurement is 711130 with a small business size standard of $11,000,000. This solicitation has documented and incorporated provisions and clauses in effect through Federal Acquisition Circular 2005-76 and DFARS Change Notice 20140805. The Contractor shall propose the following Contract Line Item Numbers (CLINs): The following are quantities for quoting purposes only. Musician services are in accordance with the Performance Work Statement (Attachment#1).
